An opportunity has become available for a Senior Mental Health Practitioner to provide specialist mental health care as a valued member of the Primary Care Network (PCN) in Four Counties. The successful candidate will be based in Lakeside Healthcare and Hereward Group Practice surgeries. It will include working with primary and secondary care and the neighbourhood teams to locally support the mental health transformation and improve the lives of people who access healthcare services.

Main duties of the job

The successful candidate will be an experienced registered practitioner with proven skills in working with people who have a range of highly complex mental health needs, demonstrating practice that is evidence-based and innovative

Key elements of the role will include (but not be limited to):

To work as an accountable and autonomous practitioner working within specified GP practices to provide specialist mental health within this primary care setting.

Plan, implement, monitor and review therapeutic interventions with individuals / carers who have serious mental illness through holistic patient centred assessments. Providing specialist clinical knowledge and interventions to service users in primary care setting. This will include working with the practice team to support people with highly complex mental health needs/serious mental illness.

Providing best practice training, mentorship and guidance to mental health practitioners and other roles within the primary care/ neighbourhood MDT

Managing a caseload of highly complex/SMI presentations working closely with the locality secondary mental health service, to maintain personalised care plans which focus on what matters to the person.

Attend core neighbourhood working meetings as part of an integrated team and works in a collaborative manner with the Neighbourhood Lead.

Facilitate, participate and optimises case conferences / multi-disciplinary team (MDT) meetings.

The role gives excellent opportunity for co-production with Lincolnshire health, social and voluntary care system, patients, and carers. This not only ensures appropriate, and needs led outcomes for the patients, but also offers the practitioner opportunities for creative approaches to working and ensuring the patient remains central in the care and intervention planning. This enables individuals to achieve their optimum physical, psychological, and social wellbeing and improve the lives of people who access healthcare services.

A comprehensive trust induction will be offered and opportunities for CPD. Management and Clinical supervision are provided regularly.

Person Specification Qualifications Essential
  • Degree in mental health and social care or equivalent
  • 1st level registration RMN
  • Evidence of specialised continued professional training (degree level) in clinical practice
  • Non-medical prescriber (or ability to complete within first 12 months in post)
  • Evidence of advanced clinical practice at post graduate level to masters or equivalent
  • Evidence of continuous professional development
Desirable
  • Management or leadership qualification
Experience Essential
  • Post-Registration experience of working with adults with mental health needs
  • Experience of working autonomously within a senior position
  • Experience of managing staff and good leadership skills
  • Experience of working across organisations
  • Sound knowledge of the national agenda for mental health
  • Sound knowledge of clinical/ risk assessment and understanding of confidentiality
  • Sound knowledge of community care issues, local health and social structures, functions and boundaries
  • Sound knowledge of the Mental Health Act

  • Experience of leading service change/development/transformation
  • Experience of leading training sessions
  • Experience of neighbourhood working
