An opportunity has arisen for a highly motivated individual to work within a dynamic and innovative Community rehab team to provide specialist individualised patient care for people living with various complex conditions, often with interesting and varied range of cognitive disorders.

The Community Independence Service in Kensington and Chelsea is looking for motivated and enthusiastic Band 6 Physiotherapist to join our friendly and fun loving rehabilitation team. You will be passionate about the value of community rehabilitation and preventing hospital admissions, have had a variety of clinical experience and be ready to step into a community role to further develop your clinical and non-clinical skills. We invite both current Band 6 and Band 5 Physio's with a view to a Band 6 post to apply.

The community setting is THE PLACE TO BE with care being prioritised at home and patients being cared for in the community wherever possible - this is clearly outlined in the NHS Long Term Plan and can already be seen on the ground.

Main duties of the job

-To be professionally and legally accountable for all work undertaken. To work within the standards and guidelines of the team, the Trust, the CSP Code of Professional Values and Behaviour and the HCPC Standards of Conduct, Performance and Ethics.- To undertake physiotherapy assessment of clients, (suitable to band & experience) with diverse needs through subjective questioning and objective assessment.- To prioritise factors involved in the clients presenting condition through clinical reasoning based on knowledge of current professional opinion, clients prognosis and other physical, social emotional and mental health issues.- To set goals for treatment with clients and/or their carer and ensure that care is client centred and agreed by the client and/or their carer.- To set and maintain a high standard of clinical practice which is evidence based, using standardised assessment and outcome measures where appropriate- To independently screen and assess multi-disciplinary referrals from a variety of sources.

Job description Job responsibilities

We are part of a wider Community Independence Service, which is also offered in Hammersmith & Fulham and Westminster. You will be connected with peers across the tri-borough to support you and to work on service improvement partnerships. You will also be working closely with our acute partners, such as Imperial College Healthcare NHS Trust and Chelsea & Westminster Hospital NHS Foundation Trust.

You will enjoy the rewards of working with a client group for whom you can make a significant difference every day, and you will experience the strong motivation of being part of a supportive multi-agency effort which includes Medical, Mental Health, Nursing, Allied Health Professional, Social care and Reablement staff.

Person Specification Qualifications Essential
  • Degree / Diploma Physiotherapy
  • Registration with HCPC as a Physiotherapist
Desirable
  • Member of CSP
  • Post-Graduate MSc degree
  • Post Registration courses e.g. ? Neurology ? Older people/elderly rehab ? Goal setting ? Student Placement Educator
  • Membership and/or involvement in a CSP Professional Network
Experience Essential
  • Previous general experience to include the following: Older people/elderly rehab, Medicine & general surgery, Falls, Dementia, Orthopaedics, Respiratory
  • Experience of teaching juniors & assistants
  • Experience of completion of audit and service developments
  • Supervision experience - rehab assistants or HCAs
  • Experience working in a multi-disciplinary team
Desirable
  • Experience of working as an autonomous practitioner in a community setting
  • Previous general experience to include the following: ? Community working ? Neurology ? Pain Management ? Group work
  • Experience of teaching / mentoring students
  • Evidence to show understanding of the goal setting process
Skills Essential
  • Demonstrates flexibility, problem solving, initiative and leadership skills
  • Able to demonstrate clinical and professional development
  • Understanding the principles of MultiDisciplinary Teamwork
  • Understanding of the problems of disability and its effects
  • Able to demonstrate a good understanding of role of the physiotherapist in rehabilitation as well as the role of other professionals in MDT
Desirable
  • Excellent IT skills including Outlook, Word, Excel, databases, etc
