New Hire Bonus £1500

The permanent posts are eligible for a £1500 New Hire Bonus. An initial payment of £500 will be paid in the first 1 month and a final payment of £1000 will be paid 1 year after your start date. This will be pro-rata for part-time posts. Internal applicants are not eligible.

The Crisis Resolution Home Treatment Team for Older People has an exciting opportunity for an experienced and dynamic mental health practitioner to join us in Beechcroft at Fulbourn Hospital site .

The team offers intensive support and treatment at home, and in care homes, for older people with mental health problems, including the behavioural and psychological symptoms of dementia and we support people who live in the Cambridge Area.

Main duties of the job

  • Crisis intervention, intensive treatment, support, risk assessment and management to prevent unnecessary hospital admissions and facilitating earlier discharge from mental health hospital beds.
  • Integrated services to promote a holistic approach to care and recovery-focused interventions for patients suffering with mental ill health
  • Dementia Intensive Support which offers a practical and a psycho-educational approach to care within patients' homes and care homes - The aim is to avoid admission to hospital from carer burnout and to prevent care placement breakdown.
  • We provide time-limited interventions and patients will be referred for longer term monitoring, support and treatment if appropriate within Older People's Neighbourhood Teams to ensure patients receive the best opportunity to promote their recovery and well-being.

  • The successful candidate will be working from the team's base at Beechcroft in Cambridge (Fulbourn hospital site) and completing assessments/ community visits within the wider areas of Cambridge.
  • We operate from 8am - 8pm, seven days a week, including bank holidays.
  • The successful applicant will be working with both patients with a functional mental health diagnosis and within our Dementia Intensive Support Pathway supporting patients with a diagnosis of dementia and their carers in the community. This may be supporting people in their homes or visiting them in care homes.
  • MHP's within the team provide assessment and interventions for people with significant mental health conditions including dementia.
  • They also offer advice and guidance to colleagues from other health and social care providers and will work alongside those services.
  • The successful candidate will participate in team meetings and present details of assessments to colleagues within the meetings.

Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot usepublic transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. Person Specification Education and qualifications Essential
  • Registered Mental Health Nurse or specialism relating to post such as Registered Nurse Learning Disability
  • Evidence of Continued Professional Development.
  • NMC registration.
  • Mentor qualification / ENB 998 or equivalent.
Desirable
  • Post graduate qualification related to specialism
  • Additional qualification for specialism.
Experience Essential
  • Experience of dealing with service users with complex mental health needs
  • Basic training in the Mental Capacity Act
  • Experience of inpatient services and working as part of a multi-disciplinary team
  • Experience of providing mentorship
  • Experience of supervising staff
Desirable
  • Lived experience of mental health
  • Experience of developing service user and carer involvement
  • Experience relating to ward specialism.
  • Experience of undertaking Continuing Care Assessments.
Skills and Abilities Essential
  • Effective communication skills both verbally and written including IT literacy
  • Ability to work as part of a team, as a role model, empowering and motivating other team members.
  • Ability to build effective working relationships with staff across a wide range of professional and managerial groups.
  • Able to offer in-service training to junior colleagues Enthusiastic and has the ability to participate in full rotational shifts.
