Stroke Physiotherapist based at Williton stroke unit, this post will initially be based on the ward and progression to the ESD and community therapy service will be made. There is also weekend working and this is managed by a self roster system.

We now have opportunities for dynamic, motivated and experienced individuals to join our Stroke and ABI ESD service. This innovative service delivers intensive rehabilitation within patients' homes using state-of-the-art mobile technology, ensuring patients are kept at the centre of all decisions, goals, intervention and discharge plans. The team consists of Occupational Therapists, Physiotherapists, Nurses, Speech and Language Therapists, Psychologists and Rehabilitation Assistants jointly providing seamless services to patients within their homes and along the whole stroke pathway.

Main duties of the job

Our service provides Early Supported Discharge (ESD) for both Stroke and Acquired Brain Injury (ABI) patients across Somerset as well as specialist inpatient and ongoing community stroke rehabilitation.

To support this service, we require a part time Physiotherapist to work as part of the team and contribute to the efficient countywide service.

Our service provides Early Supported Discharge (ESD) for both Stroke and Acquired Brain Injury (ABI) patients across Somerset as well as specialist inpatient and ongoing community stroke rehabilitation.

The therapy service provides intensive stroke rehabilitation to patients on the ward on our stroke rehabilitation units and also in the community on our early supported discharge service. This allows patients in their own homes to access the early intensive therapy they may need on discharge from hospital.

The team comprises of a mutli discplinary approach with nurses, rehab assistants, speech therapy, occupational therapists and assistant practitioners.

Person Specification Qualifications and Training Essential
  • Diploma/degree in Physiotherapy
  • HPC Registration
  • Evidence of CPD maintained in a portfolio including attendance at recent postgraduate courses
Desirable
  • Membership of the Chartered Society of Physiotherapy
  • Post graduate accredited course in Stroke rehabilitation Care of the Elderly and/or
Knowledge & Experience Essential
  • proven post graduate experience with extensive clinical experience relevant to the post.
  • Experience of a wide range of clinical rehabilitation areas of practice to include Stroke, elderly patients with complex multipathology, neurological, orthopaedics etc
Desirable
  • Experience of working as part of a multidisciplinary team
  • Experience in a Community Hospital setting
  • Experience of lone working
  • Experience of service development and review
