As part of the ongoing transformation of Mental Health service provision we are recruiting Primary Care Mental Health Practitioner posts across the full Mersey Care footprint to be deployed in primary care networks in Liverpool, Sefton and Kirkby. These roles will be based directly within GP practices, providing a preventative and 'hands on' approach to supporting GPs in the provision of mental health care.

You will work as part of the primary care team, covering all the GP practices within a designated Primary Care Network, adopting a population health approach to ensure the needs of the community are met.

Main duties of the job

This role is an instrumental part of the wider community transformation programme and you will receive supervision and support from a Mersey Care Mental Health Lead covering the same geographical region as you. You will also receive day to day support from a Primary Care Lead within your designated Primary Care Network.

This is a brand new role and there is a requirement to be adaptable, creative, enthusiastic and dynamic. It is an exciting opportunity for innovative working across community providers to improve the quality of mental health services and address health inequalities. The role will evolve in accordance with the needs of the local population and your primary focus will be to ensure that GP practices are supported in the management of people who present to Primary Care with mental health difficulties.

Job description Job responsibilities

• To have a physical presence in surgeries, building direct relationships with GPs & PCN based staff
• To see mental health patients who frequently attend GP practice
• Support assessment and coordination of next steps for mental health patients attending GP practice in crisis
• Proactive signposting to local community resources, including Life Rooms and VCSE providers
• Participate in consultations with GP to provide additional clinical and system navigation expertise
• Support completion of physical health checks & interventions for people on the SMI register
• Support management of patients who have been discharged from secondary care and require additional mental health input within GP practice
• To provide interventions focused on the needs of the local population. For example, coordinating group interventions for people with EUPD in an area that has a prevalence of this condition
• Participate in Primary Care MDT team meetings, assisting case reviews and decision making about Mental Health patients
• Completing all necessary clinical documentation

Person Specification Qualifications Essential

• oAppropriate professional qualification or experience at Degree Level e.g. RMN, AHP, Social Worker
• oEvidence of post registration study / continuing professional development e.g. Mentorship in Practice

Experience Essential

• oMinimum of 2 years post registration experience in mental health.
• oEvidence of experience of managing cases & case loads
• oAbility to work autonomously across the organisation/boundaries and with other agencies
• oExperience of working in partnership with other agencies and service users and carers
• oEvidence of experience of working with a range of people with mental health problems in a variety of health care settings
• o A demonstrated understanding of Equality and Human Rights
