We are pleased to offer the new and exciting opportunity to join the new Mid and South Essex, Older Adult Admission Avoidance & Home Treatment Team. As part of the transformation agenda a new service is being formed to provide support to Older Adults, with Frailty and Functional Mental Health needs.

Main duties of the job

As part of this developing service you will help support older adults with functional mental health issues to meet increased needs / rising risk to support admission avoidance and provide home treatment with a Home First approach to support. You will work with a new and growing team, the existing community services and the established Dementia Admission avoidance services across Mid and South Essex.

Job description Job responsibilities

To work as a senior Occupational Therapist delivering Occupational Therapy interventions to Older People in the Community to support admission avoidance and provide home treatment. Responsible for ensuring that the values outlined in the NHS Constitution are adhered to daily and any matters of concern are raised with the relevant Line Manager or through the necessary processes within the Trust.

Responsible for delivering a compassionate, dignified and respectful service to patients at all times.

Person Specification Qualifications Essential
  • Registered Occupational Therapist.
  • Degree or Diploma in Occupational Therapy.
  • Additional relevant qualifications, e.g. counselling, dementia care mapping, normal movement, etc.
Knowledge Essential
  • Appropriate assessment techniques.
  • Advanced/working knowledge of pathology and occupation in its widest sense, e.g. Mental Health, musculoskeletal, neurological, sensory impairment, the impact of the aging process on function.
  • Theories of group work.
  • Research methods including critical evaluation of evidence.
  • Models of supervision.
  • Learning theory.
  • Working knowledge of the prescription of aids and adaptations to independent living.
  • Risk assessment particularly in relation to home assessments.
  • Specialist assessments.
  • Reminiscence Therapy.
  • CPA process.
  • Validation Therapy.
  • Reality Orientation.
  • Model of Creative Ability
  • PAL activity levels.
Experience Essential
  • Experience of working with people with Mental Health problems OR experience of working with older people OR experience of working with neurological problems.
  • Group work.

  • Membership of Special Interest Group.
  • Supervision of students.
  • Supervision of unqualified staff.
  • Working knowledge of Mental Health legislation.
  • Manual Handling assessments.
  • Working in Social Services.

  • Utilisation of appropriate assessment techniques to assess individual's abilities in relation to their desired goals in everyday activities including mobility.
  • Application of advanced/working knowledge in order to plan and achieve realistic outcomes for individuals and groups - physical and mental health.
  • Utilise interpersonal skills which motivate individuals including those with cognitive impairment and their carers to take control of their everyday activities of self-care, leisure and quality of life.
  • Apply a variety of treatment modalities in group and one to one format.
  • Work with and through others to achieve environmental modifications that empower individuals and carers to achieve independence.
  • Integrate theory and practice and demonstrate creative problem-solving processes.
  • Critically reflect on and use evidence in order to develop best practice.
  • To work and manage self independently.
  • High standard of communication skills written and verbal.
