This is an exciting full-time opportunity for both the experienced Advanced Assessment Nurses and highly specialised physiotherapist to join a dynamic and innovative team providing holistic assessment and treatment in a variety of community and acute settings designed to reduce preventable hospital admission. The referral source will typically be from general practice, NHS 111, A&E, ambulance services or community- based health and social care (including care homes), Acute Trusts and other rehabilitation in-patient centres.

Main duties of the job

The aim of the service is to avoid unnecessary hospital admission by:
  • Providing the Hillingdon borough population with an alternative to hospital admission through rapid deployment of clinical response to patients in their own homes.
  • Providing a rapid response (2-hour response to referral, within operating hours) to make initial assessment and form onward plan.

The service runs from 08:00-00:30 including weekends and bank holidays. Typical shifts run from 09:00 - 17:00 and 16:30 - 00:30. Successful post holders must be able to work across 7 days per week in a full-time post. As well as working in the community work will include working in A&E, Discharge to Assess and shifts may include 8am-8pm. Successful candidates may be expected to support Community Post Covid MDT service.

About us

Candidates should be experienced and have a sound knowledge of the community. Will need the ability and clinical skills to work autonomously but to also work alongside other members of the multidisciplinary team.

The team is comprised of nurses, physiotherapists, occupational therapists of varying experience and support staff (including Nursing Associate), with access to GPs, pharmacists, case managers, social services, dietitians as well as other specialist nursing and therapy services located in the borough.

Candidates will have undertaken professional development and have extended clinical skills including advanced physical assessment skills or willingness to undertake. The post holder will be expected to identify and promote areas for personal and service development.

Possession of a UK driving license and access to a vehicle for business purposes is essential.

Person Specification Qualifications Essential
  • RGN / Degree/diploma in physiotherapy
  • At least one clinical post registration qualification i.e. oSpecialist Practitioner oPhysical assessment course oRecognised diploma oITU, CCU, A and E oNurse Prescribing Course
  • Assessor/Mentor
  • State Registered
  • Degree/Diploma
  • Member of CSP
  • Relevant post-graduate qualification oPhysical assessment course oNurse Prescribing Course
Experience Essential
  • Multi-agency working
  • Experience of sensitively managing & resolving complaints.
  • Experience of managing others including PDP & appraisal.
  • Extensive District Nursing experience /or other comparable/transferable
  • Extensive post-graduate previous experience in: Intermediate care, Community, Elderly care, Neurology and Orthopaedics
  • Experience in supervising staff
  • Experience in working in a multi-disciplinary team
  • Knowledge of NMC / NICE Guidelines & current Government thinking.
  • Experience of sensitively managing & resolving complaints.
  • Audit experience
  • Project Management
  • Managing a delegated budget.
