To offer an extended service to ensure an appropriate assessment of mental health need for CYP in mental health crisis and in need of appropriate intensive support ensuring the delivery of safe and effective practice as part of the CYP wellbeing and recovery action plan. The post holder will work directly with children and young people with mental health needs and their families, carrying out assessments (including risk management) and effective treatment and/or interventions, with theaim of helping children and young people participate in normal family and community activities and to facilitate positive outcomes of care in relation to young people's mental health needs. The role will also facilitate hospital admission avoidance and seamless transitions between services whereappropriate. The post holder will participate in the delivery of the agreed aims and objectives of the service and identify factors which may inhibit these from being achieved. They will assist in the delivery of ahigh standard of professional care to service users referred to the team. They will implement agreed systems for the monitoring of effectiveness of care. They will participate in the education and management of junior staff.They will also be expected to work flexibly within the Team to support a crisis, brief intervention and intensive support function.

We are actively seeking applications for band 6 posts from staff with a professional qualification in Mental Health Nursing. Experience in working with CYP in Mental Health, Autism or Learning Disability services would be beneficial

About us

1. Delivery of holistic assessment, intensive support and treatment for the identified community client group and within a multi-disciplinary team.2. Support for hospital admission avoidance and facilitation of, timely discharge from hospital.3. This is an extended service, 8am - 10pm, which will involve shift working, including weekends and bank holidays4. To coordinate the workload of the team on an as required basis.5. To provide an active involvement in the assessment, development, implementation and evaluation of CYP care which focuses on ensuring that CYP are supported to remain in their own homes wherever hospitalisation can be safely and appropriately avoided.

Person Specification Qualifications and Training Essential
  • Registered Mental Health Nurse with current NMC registration
  • Post-registration experience, evidence of formal courses and training at diploma/degree level
  • Demonstrable knowledge and continuous post qualified experience/professional development
  • Conforms NMC requirements
Desirable
  • Recognised appropriate qualification in mentoring/assessing/supporting students in placement
  • Qualification in providing supervision
  • Non - medical Prescriber
  • Principles of Physical Healthcare Assessment ( or equivalent)
  • Psychological intervention/Professional Qualification including, Motivational Interviewing techniques CBT, CBTp, Family Interventions, CAT, DBT, Solution focussed interventions, Positive Behaviour Support Planning,
Experience Essential
  • Evidence of extensive post registration experience with working with those who may experience complex mental health needs in the community setting Experience of working with people who have difficulty in engaging with services and have complex mental health needs
  • Experience of making decisions autonomously whilst working with people and families in the community setting
  • Experience of assessing, planning, co-ordinating and managing care for those with mental health
  • Experience of integrating care delivery across multi-disciplinary /multi agency teams
  • Experience of assessing and managing complex needs including risk within a structured framework such as CPA/Safeguarding Children frameworks
  • Experience of effectively managing mental health related issues/referrals/need for advice face to face and virtually, including telephone, at times of high demand and in relation to highly stressful/emotive situations
  • Experience and ability to carry out clinical audit
Desirable
  • Experience in CAMHS although the lack of explicit CAMHS experience is not an excluding factor
  • Experience of working with people who may have neurodevelopmental needs such as Autism, ADHD or Learning Disability
  • Experience of working in a crisis/intensive support model
  • Experience of presenting and educating other services at local and/or national level
