Essex Learning Disability Partnership are looking for a Registered Health Professionalwith a passion for delivering the highest standard of care for individuals that have a learning disability.

You will be working as a Enhanced Support Nurse Practitioner within the Mid West Essex Community Learning Disability Team, based at Technicon House in Braintree.

Our Enhanced Support Service provides intensive support to adults with learning disabilities who are presenting with challenging behaviour and / or mental health problems in the community to prevent, where possible, admission to hospital. The team acts as a gateway to the inpatient assessment and treatment service and works in close liaison with inpatient services to facilitate appropriate admission and timely discharge from services. The team provides an 8am-8pm service Monday to Friday, 9am-5pm Weekends and Bank Holidays and works with service users in a variety of health and social care settings. The service is committed to improving the health and well-being of people with a learning disability across Mid West Essex, by working in a simple, person-centred way that empowers individuals to maximise their potential.

Main duties of the job

The Role:

• Working as a practitioner with a special interest in challenging behaviour and mental health wellbeing for people with learning disabilities.
• Assessing the health care needs of people with Learning Disabilities in a range of environments and implementing interventions as required.
• Working in an interdisciplinary way to assess service user's needs, devising care plans and evaluating outcomes.
• Working directly with people who use our services, their carers, colleagues and other service providers.

The Requirements:

• Professional Registration with Nursing and Midwifery Council (NMC) or Health & Care Professions Council (HCPC).
• Compassion, resilience and a dedication to empowering and supporting the independence of adults with learning disabilities who use our services.
• Demonstrable experience of caring for people with learning disabilities or complex health needs in a way that is most appropriate for their needs.
• Knowledge of common physical, mental and emotional health needs experienced by people with a learning disability.
• Experience of communicating calmly, efficiently and in a friendly manner, using a variety of methods, with people who require support, members of the MDT, carers/family and other agencies involved in the person's care.
• Experience of maintaining high standards of record keeping.
• A good understanding of legislation and national guidance relating to the care and support of people with a learning disability.
