We are currently seeking a full-time qualified Nurse or Occupational Therapist to join our Primary Care Mental Health Service based in Watford and covering Watford and Borehamwood/Potters Bar.

The Primary Care Mental Health Services are a new service providing interventions to those not meet the criteria for secondary mental health services but may need more than is offered by Improving Access to Psychological Therapies (IAPT) Services.

The nurse will also assist in the delivery of various group and 1:1 interventions which are evidence-based to equip those with moderate and severe mental health distress to devise recovery plans. The post holder will also work with service users who have 'stepped down' from community mental health teams and require some ongoing time-limited interventions.

The successful applicant will offer either a nursing focused or occupational therapy focused interventions. Successful nursing applicant will lead on the provision of interventions for those who physical and mental ill health affect each health concern. Successful occupational therapy applicant will lead a service focused on occupation, occupational identity and occupational recovery.

Main duties of the job

This is a Band 6 mental health practitioner post. You will have a relevant professional qualification (nursing, occupational therapy), and experience of working with service users with mental health needs.

A nurse applicant will be willing to lead groups and run physical health clinics and support interventions that address physical and mental ill health issues. There will the opportunity to work on a 1:1 basis with service users.

An occupational therpy applicant will lead the occupational therapy provision, setting up groups as necessary and working with individuals to rebuild their occupational performance and sense of meaning that comes from occupational identity and productivity.

Job description Job responsibilities

Nurses
  • To provide signposting advice that directs service users to other services and to undertake short term interventions for clients with mild, moderate and severe mental health difficulties.
  • The post holder will contribute to ensuring systems are in place to promote smooth pathways between primary care and secondary mental health services.
  • To work in partnership with service users with complex health presentations, developing their care plans and recovery plans to enable them to lead fulfilling lives.
  • To work in partnership with health and social care professionals from other agencies and organisations including Community Navigators
  • To demonstrate an understanding of clinical interventions that include psycho-education, relevant aspects of social inclusion, recovery, care plans and discharge planning, working closely with carers, families and partner agencies to facilitate this.
  • The post holder will work at all times to promote the safety and the well being of service users and their families/carers.
  • To work with PCMHS colleagues in provision of consultation, support, education and problem solving approaches to GP surgeries particularly in the area of management of mental health
  • The post holder will be an autonomous practitioner who will support the transition of mental health service users from secondary care to primary care.
  • The post holder will participate in a team caseload rather than an individual caseload.
  • To provide leadership, management and supervision for junior staff identified to support the EPCMHS.

Clinical Responsibility
  • To use specialist mental health knowledge to deliver interventions to service users which include physical health, drug and alcohol harm minimization and management of mental ill health and distress.
  • To develop links and foster a culture of partnership between primary care services and HPFT Adult Mental Health Services.
  • To promote the use of treatment protocols and care pathways resulting in smooth transition between services.
  • Facilitate joint work between primary care and secondary care and voluntary sector to increase range of services/interventions available.
  • To contribute to the building of close relationships with primary care GP surgeries that supports smooth pathways for service users.
  • To work in partnership with the individual service user and their families and carers as appropriate to enable the development of a plan to facilitate their safety, promote their wellbeing and support their independence and inclusion in the community, by using recognised age appropriate self management tools where applicable. This may include signposting to partner agencies such as voluntary sector agencies, schools and colleges which may be able to more appropriately help the service user complete these tools.
  • To discuss with the service user and when appropriate, with their carers, their health problems and how they see and understand them, facilitating a process of developing their understanding and maintaining their hope for the
  • To promote the empowerment of individual service users to manage their health as much as possible and signpost use of Personal Budgets as indicated.
  • To provide advice and support to and family members if relevant.
  • To participate in the development of pathways that work with primary care to utilize the primary care registers for service users with a serious mental illness or on medication for mental health e.g. depot, lithium that ensure they receive physical health checks and reviews of their medications.
  • To explain and discuss medication with the service user, including side effects they may experience and the role that the medication may have in their recovery or ongoing care.
  • To work closely and in partnership with colleagues within primary care and the Trust in a collaborative way, ensuring that decisions are made that ensure the best care for service users and that there is a seamless pathway.
  • To develop professional networks and keep abreast of local resources; third sector partners, primary care, housing, education, social care and other statutory organisations in order to ensure that the service user is able to navigate the full health and social care system.
  • To establish robust communication networks with other health workers and
  • To demonstrate effective communication skills with people in an age appropriate way and those whose understanding may be impaired.
  • Communicate effectively with junior staff on a timely basis ensuring systems exist so that staff, are informed.

Occupational Therapists
  • To manage a caseload of service users with complex OT needs, using evidence based/service user-centred principles to assess, plan, implement and evaluate interventions in clinic and community settings and on home visits.
  • To provide leadership for junior staff, through supervision and appraisal
  • To participate in the planning, development and evaluation of OT services, holding responsibility for defined projects where appropriate
  • To regularly supervise OT students on practice placement, following attendance at an approved Fieldwork Educators course
  • To apply knowledge of the Model of Human Occupation (MOHO) and other standardised assessment approaches relevant to the clinical setting.
  • Following assessment, to undertake case formulation in order to highlight occupational strengths and limitations and to inform the intervention plan moving forward.
  • To work collaboratively with service users to identify OT goals as part of the overall recovery plan.
  • To provide interventions in the manner that is consistent with Trust standards and values.
  • To routinely provide feedback to the MDT regarding observed occupational performance throughout contact, which inform ongoing assessment and therapeutic intervention.
  • To provide specialist support and education to service users and carers regarding aspects of occupational therapy e.g. equipment fitting, rehabilitation programmes.
  • To maintain clinical records using the electronic patient record system.
  • To liaise with carers and other professionals as relevant.
  • To exercise good personal time management, punctuality and consistent reliable attendance.
  • To attend mandatory training.
  • To comply with the RCOT Code of Ethics and Professional Conduct and national and Trust policies
  • To review and reflect on your own practice and performance through effective use of professional and operational supervision and appraisal.
  • To maintain a professional portfolio for CPD, recording learning outcomes through participation in internal/external development opportunities.
  • To actively participate in the Trust OT CPD development programme, as a learner and a facilitator.

Clinical Responsibility
  • To work effectively as a member of a team in providing an occupational therapy service to the Primary Care Mental Health Services.
  • To provide specialist Occupational Therapy advice, intervention and support to service users and their carers.
  • To provide timely feedback to the MDT information regarding the service users wellbeing and occupational performance.
  • To provide a detailed and specialist occupational therapy report when required.
  • To use evidence-based practice to engage service users in meaningful and appropriate activities, to promote independence, recovery and well-being.
  • To have a senior oversight of the needs of the service user group and develop services accordingly.
  • To demonstrate and apply a specialist level of understanding of the effect of disability and recommend adaptations to the service users physical and social environment.
  • To ensure that up to date written and electronic records and activity data are maintained in accordance with Professional and Trust standards.
  • To undertake risk assessments within clinical area as guided by local/Trust policies.
  • To report incidents, safeguarding concerns, complaints and compliments.
  • To actively participate in safety huddles and de-brief sessions as appropriate.
