The team will provide an active involvement in the assessment, development, implementation and evaluation of service user care which focuses on ensuring that the service users are supported to remain in their own homes wherever hospitalisation can be safely and appropriately avoided. The team will deliver home treatment 24/7 to an identified group of children and young people in Crisis by responding quickly to those referred with urgent and high-risk mental health needs, and provide collaborative, effective, evidence based interventions at the right time with the right person. This team will work within a multi-disciplinary framework, and work closely with other pathways and colleagues to ensure smooth transitions take place, and this includes transitions to adult services or alternative specialist providers and support for and facilitation of early discharge from hospital

There will be scope for creativity in this team, to develop evidence based interventions, to think systemically and work collaboratively with children, young people and families, informed by the local Transformation Plan, Future in Mind and the THRIVE framework. Main duties of the job

Note: If you are currently working at an AfC Band 5 position, you are encouraged to apply for this role as part of a development programme.

Full-Time: 37.5 per week (you will be required to cover a 24 hr rota on a shift basis)We are a team of enthusiastic and experienced professionals providinga 24 hour Crisis Home Treatment Team. These Community Mental Health practitioner posts will join our existing Multi-Disciplinary team of professionals, headed by the Service Manager, Operational Leads, Clinical Leads. The BeeU service is a partnership between Midland Partnership Foundation Trust, The Children's Society, Kooth and Healios working together in the interests of children and young people in Shropshire and Telford & Wrekin.

You MUST be a fully registered Nurse, Social Worker or Allied Health Care Professional with appropriate mental health experience to apply.

Job description Job responsibilities

Main duties and responsibilities1. Provide specialist assessment and care planning in conjunction with service users andcarers, evaluation of individual treatment plans and discharge planning2. Contribution to the delivery of home treatment through nursing care to an identifiedcommunity client group and within a multi-disciplinary team.3. Support for and facilitation of, early discharge from hospital.4. This is a 24 hour service which will involve shifts including weekends and nights.5. To coordinate the workload of the team on an as required basis.6. To provide an active involvement in the assessment, development, implementation andevaluation of service user care which focuses on ensuring that service users are supported toremain in their own homes wherever hospitalisation can be safely and appropriately avoided.7. To contribute to the clinical service development as part of the 0-25 Emotional Health andWellbeing Service.8. To be responsible for an agreed caseload to be determined by the operational lead andreviewed under Clinical supervision.9. Triage and assess referrals received by service in liaison with multi-disciplinary colleagues asappropriate. Formulate and action appropriate response based on need, risk and urgency.Respond to and inform referrers/other professionals and responsible adults as necessary ondecisions, plans, policy and best practice. Record this on Trust systems (RIO andSafeguard).10. Take responsibility for the management of a complex caseload, which includes, specialistassessment (within service specification), care-planning in conjunction with service user andcarer, evaluation of individual treatment plans (Care Programme Approach and SingleAssessment Process).

Person Specification Qualifications and Training Essential
  • Registered mental health Nurse or Allied Health Professional on NMC or HCPC Register
  • Post-registration experience, evidence of formal courses and training at diploma/degree level
  • Conforms to NMC/HCPC requirements
Desirable
  • BSc degree in nursing/relevant professional degree or equivalent level
  • Qualification in providing supervision
  • Training or relevant experience in managing suicide risk and risk management
  • Training or a sound awareness of common physical health conditions and their relationship with mental illness and wellbeing
  • Registered Sign off mentor for nurse students (nurses only)
Experience Essential
  • Experience with working with children & young people experiencing mental health related difficulties
  • Experience of working with people who have difficulty in engaging with services and have complex mental health needs
  • Experience of working in community setting and making decisions autonomously
  • Experience of assessing, planning, co-ordinating and managing people with mental health problems in a community setting
  • Experience of integrating care delivery across multi-disciplinary /multi agency teams
  • Experience of leading/contributing to CPA/Care Planning process
  • Experience of dealing with mental health related issues/referrals/need for advice by telephone including at times of high demand and in relation to highly stressful/emotive situations
  • Experience and ability to carry out clinical audit

  • Experience of presenting at local/national level
