We now have an exciting opportunity for a full time Specialist Speech and Language Therapist to join our friendly Southport & Formby Community Neuro Rehabilitation Team.

The post holder will be part of a multidisciplinary team composed of Occupational Therapy, Physiotherapy and Vestibular Physiotherapy within the Southport & Formby Community Neuro Rehabilitation & Speech Language Therapy Team. The service aims to provide seamless care through community/home based rehabilitation to patients presenting with both neuro and non neuro speech and language therapy needs which may include the areas of dysphagia, dysfluency, head & neck and voice.

Main duties of the job

The main duties will be mostly clinical and post holder will be responsible to assist clinical lead with coordination of the Speech and Language Therapy (SLT) service, ensuring:

efficient management of clinical caseload and the waiting list is kept to a minimal;

demonstrate expert knowledge and clinical skills (including experience with video-fluoroscopy) ensuring it is in line with recent guidelines and evidence base practice; to use a variety of assessments skills and ability to evaluate treatment and set smart goals; to manage a complex caseload and be proactive to seek peer support when required;

to demonstrate multidisciplinary approach and respect the views of other MDT members;

demonstrate knowledge of different outcome measures and its application; to carry out complex risk assessment ensuring ability to sign post/advice/handle safeguarding cases/situations effectively and to demonstrate excellent documentation skills and ability to evaluate plan.

Job description Job responsibilities

Clinical1. To participate as a Specialist Speech and Language Therapist in a multi-disciplinary team, with specific responsibility to assess and treat adults with complex health needs.2. To be responsible for the delivery of specialist assessment, diagnosis, treatment, delivery and evaluation of evidence-based Speech and Language Therapy intervention in the community setting, including home setting, care homes or outpatient clinics for clients with communication and swallowing disorders, such as dysphagia (neuro and non-neuro), dysfluency, voice, head and neck.3. To act as a Care coordinator, ensuring that clients have full access to any generic services that they require.4. To be responsible and accountable for their own caseload and ensure that care plans are written and reviewed regularly.5. To contribute to the prioritisation of the waiting list for Speech and Language Therapy.6. To complete intake assessments on behalf of the team and feedback to the multi-disciplinary team.

Person Specification QUALIFICATIONS: Essential
  • Accredited Speech and Language Therapy qualification (BSc)
  • Post graduate training/qualification in Dysphagia
  • Two years experience post qualification
  • A philosophy and approach that empowers people
  • Knowledge of alternative and augmentative communication and dysphagia
  • A person-centred approach to service delivery
  • Membership of Health Professionals Council
  • Understanding of current NHS changes
  • Experience of working as part of a multi-disciplinary Team
  • Experience with provision of supervision and delegation to junior members of staff
  • Experience of use of outcome measures for clinical evaluation
  • Direct experience of working with adults with communication and Swallowing disorders
  • Experience working with clients with long term neurological conditions
  • Experience in Rehabilitation setting
  • Experience of inter agency working
  • Experience of designing and delivering training for staff groups
SKILLS Essential
  • A demonstrated understanding of Equality and Human Rights
  • Ability to convey information and skills to direct care staff
  • Good time management and organizational skills
