We are looking to recruit a Band 6 CPN with significant experience of working with vulnerable groups with complex needs to join the Homeless Rough Sleeper Team who work in partnership with other commissioned statutory and third sector homeless organisations to deliver a holistic package of care to entrenched Rough Sleepers in Birmingham, in order to enable them to access accommodation and support for their complex lives to allow them the opportunity to change their lifestyle and lead healthier lives.
An ability to make relationships, provide sustained support to non-engaged individuals and work in sometimes difficult conditions are required and in return the post holder will be working as part of a NHSE project to end street homelessness in Birmingham and change the most vulnerable members of our community's lives for the better.
The team is based in a variety of venues, mostly within Birmingham city centre alongside colleagues from other organisations, the service will cover the hours of 6.30am to 5pm Monday to Friday (ability to work flexibly). The team has a strong clinical network including input from a consultant psychiatrist, senior psychologists, a clinical lead/nurse prescriber, CPN's and administration support with an emphasis on multidisciplinary working, case management and a team ethos to support a small number of individuals, whose lives this service is hoping to change for the better.

Main duties of the job
The post holder:
- Will act as an independent practitioner within the community setting, while liaising with other professionals as necessary
- Will manage a defined caseload and provide supervision to others if appropriate.
- Will be responsible for the assessment, planning, implementation and evaluation of nursing care within agreed parameters, and have a working knowledge of local, national policies and legislation, which govern current service provision and audit.
- Will be responsible for ensuring effective communication pathways exist.
- Will be responsible for ensuring the promotion of equality, diversity and rights in accordance with good practice and legislation.
- Will be responsible for identifying and contributing to the training needs necessary in providing quality evidence based care as appropriate.
- Will be responsible for actively prescribing within a defined area and for a defined group of service users
