As part of the Community Transformation, we are seeking to recruit an Advanced Nurse Practitioner to work in the newly developed HUB.

The role is looking towards enhancing and leading the nursing team within the new HUB - working closely with all NHS and other agencies to develop a collaborative approach to patient care. The new ways of working will work closely with the service users and their families to ensure they receive the right care from the right service in a timely manner.

We are looking for someone who will enjoy a challenge, remains calm under pressure, is enthusiastic and can make a difference.

The ideal candidate will be an excellent communicator, committed to providing a high-quality services and dedicated to enhancing the clinical role.

Main duties of the job

This is a great opportunity to develop the role of the Advanced Practitioner in the HUB and be part of the changing face of mental health services. If you want to make a difference to patient care and involve them in transforming services to create a shared approach, then this is the job for you.

In the Hub you will be active in the front end of services, involved in triage, assessment, short term interventions, but also part of the monitoring of long-term care of patients on specific medication.

The Advanced Nurse Practitioner role is looking towards enhancing and leading the nursing team, alongside the leadership team - looking at developing skills and knowledge.

  • Qualification in non-medical prescribing or willingness to undertake within agreed timescale
  • Teaching in clinical practice certificate
  • Recognised sign-off mentor or willingness to work towards. Must be achieved within agreed timescale
  • Key skills in literacy, numeracy and ITQ level 2 (or equivalent)
Desirable
  • Clinical leadership qualification
  • Masters qualification in a subject relevant to the area of practice
