We are seeking a motivated health care professional who wants to shape services for individuals with dementia and mental health problems living in care homes. This new post will provide a conduit between GP's, care homes and specialist mental health services to ensure high quality patient care through collaborative working. The post holder will be leading on the scoping of provision of mental health services within the Enhanced care home offer from G.P's and will co-produce a clinical model with partner agencies.

Main duties of the job

The successful candidate would provide a lead role in the scoping and development of the Enhanced Care Home Mental Health model and influence strategic development in response to changing client, local community and national needs. They would be responsible for the implementation of the initiative, alongside the evaluation and monitoring of any changes. Dissemination to the wider team and partner agencies regarding the initiative would also be expected.

The candidate would provide a navigation role to assist GP's to make appropriate referrals to mental health services, for individuals living in care homes and to aid in partnership working across the city. This would provide a co-ordinated and integrated approach to ensure effective working relationships exist between GP's and mental health teams. The post holder should have significant clinical expertise to provide clinical input with complex cases and to provide education and training for patients, carers, staff and partner agencies.
