Main duties of the job

You will be part of the friendly and supportive Stroke Rehabilitation services (Sneyd Rehab Unit & Integrated Community Stroke Service) where there is a wide range of expertise and experience. We strive to support our patients within our nationally recognised bed based specialist unit and community; facilitating a smooth and timely transition from hospital to home optimising stroke recovery, function and independence post discharge home from hospital.

The role provides opportunity to further develop your specialist clinical and leadership skills in this specialist area. This is a varied role and responsibilities will include specialist assessment, therapy and management for our stroke population. The successful applicant will be a dysphagia competent clinician, with experience in working with adults with an acquired range of swallowing and communication difficulties who is willing to work flexibly with colleagues to meet service need. There will be a rotational element to the post,

This role is mainly clinical facing but may include some innovative remote working and virtual appointments. Domiciliary visits to patients' homes, community hospitals and care home settings will require the post holder to travel independently.

Service development, training and research opportunities alongside excellent peer support will be available as part of the Haywood Hospital Therapies Portfolio and the Adult SLT team.

You will need to have the following skills or experience:
  • Be a registered Speech and Language Therapist
  • Be an independently practicing dysphagia clinician (having completed and passed the RCSLT accredited post graduate dysphagia training)
  • Active participation in service and staff leadership, training and service improvement
  • Outstanding communication skills, high emotional intelligence and a resilient, problem-solving approach.
  • A willingness to undertake post-registration training relevant to the post
  • A willingness to attend Clinical Excellence Networks relevant to the post

Main responsibilities:
  • To assess, diagnose, treat and manage a specialist caseload of clients and maintains associated records as a specialist member of the Speech & Language Therapy profession within our Stroke Services.
  • To work with colleagues in an Interdisciplinary way for the benefit of stroke patients and the service, towards key service and National objectives.
  • To train and support the carers of clients and participate in appropriate specialist training to other colleagues.
  • To advise and provide clinical support to other therapists/staff on Speech and Language Therapy issues.
  • To supervise the work of Rehabilitation Support Workers, students and other staff working with the postholder.
  • To provide specialist intervention and evaluate outcomes
  • To work in partnership with colleagues across the stroke rehabilitation pathway and take part in service improvements locally and as part of the ISDN across the wider regional Network.

CLINICAL
  • To adhere to and apply the Health and Care Professions Councils (HCPC) standards of proficiency and the Royal College of Speech and Language Therapists (RCSLT) code of ethics and professional conduct and all other relevant guidance from MPFT. To be professionally accountable and responsible for all aspects of own work.
  • To establish and maintain effective working relationships both within MPFT and with external organisations in order to meet service requirements and support and demonstrate the Trusts equality values.
  • To assess new service user referrals and care/support needs. Assesses service users and formulates, develops, implements and evaluates programmes of treatment /care/support for individuals, families and carers. The care/support plans may include the following:
    • Promoting independence/self-management and maintaining safety
    • Increased functional ability
    • Improvement in health and wellbeing including mental health
    • Interventions in disease/condition management
    • Prevention and reduction of health inequalities
    • Admission avoidance
    • Early supported discharge
    • Case management/ treatment/care plans
    • Palliative and end of life care
  • To use professional knowledge and judgement to take into account the range of care/support packages and treatment options available. To consider each service users personal circumstances and preferences in order to implement and adjust programmes of care and intervention.
  • To maintain and develop skills and knowledge to deliver a range of interventions and sets an example of good practice to other team members including skill sharing. To assess, diagnose and manage complex/specialist communication disorders and/or dysphagia. To use highly developed auditory and perception skills in transcribing and analysing service users speech. To provide evidence based outcomes following assessment and intervention.
  • To maintain sensitivity at all times to the emotional needs of the service user and their carers offering reassurance and empathy, in particular when imparting potentially distressing information. This may be in regard to the nature/prognosis of their condition and implications of the same.
  • To work with individuals and carers who may find it difficult to engage with the service or aspects of the service resulting in uncooperative or challenging behaviour that has to be managed.
  • To use professional knowledge, skills and expertise when working in changing environments and across professional boundaries.
  • To maintain contemporaneous and accurate records as required by the Partnership Trust and in line with professional standards (i.e., HCPC). Support identified team members giving guidance and direction where appropriate.
  • To take responsibility for the safe use and transportation of equipment between locations. To work in non-purpose built rooms involving the necessity to move furniture, therapy equipment etc.
  • To ensure that the skills of identified team members are appropriately deployed and staff with the relevant education, expertise and competence deliver that care.
  • To deliver a high standard of intervention, whilst managing the demands of unplanned aspects of the role using innovative problem solving techniques.
  • To provide advice, education and training to service users, carers and professionals regarding the management and care/support of service users.
  • To provide professional and clinical support and leadership to identified team members. Supports in identifying training needs of staff and encouraging practitioners to develop skills in:
    • Critical thinking and reflective practice
    • Excellence in practice
    • Specialist knowledge and research based practice
  • To take accountability for allocated assessments, planning and implementation of care packages/programmes of support, within their caseload

  • SERVICE DELIVERY RESPONSIBILITIES
    • To contribute to the design and implementation of measures to monitor and develop services including meeting high quality standards of care as set out in national and local policy, including that of the RCSLT, HCPC and CQC.
    • To alert relevant line manager to gaps and clinical risks and other pressures on the service, proposing remedial measures when required.
    • To understand the teams policies, structures and objectives.
    • To produce written documents and other forms of presentation to communicate complex and sometimes sensitive information for a variety of recipients (e.g. leaflets, persuasive presentations to managers, evidence for educational or disciplinary tribunals).
    • To contribute to the development and monitoring of an effective Clinical Governance programme in conjunction with other team members and to promote best practice through participation in evidence-based study.
    • To design, deliver and evaluate quality training programmes for therapists on the topics of her/his specialism/s and to include and meet the needs of allied professionals in some of these events.
    • To promote effective, collaborative working partnerships with other agencies, including initiating and supporting joint team projects.
    • To be prepared to present on specialist area to larger audiences, (e.g. presenting research papers at conference, leading training sessions).

    MANAGERIAL RESPONSIBILITIES
    • To provide specialist advice and clinical supervision for less experienced therapists and others involved with the client, e.g. professionals/carers.
    • Working with the Clinical Leads to co-ordinate changes/improvements to services and work under the guidance of the Service Manager.
    • To supervise SLT students placed in own area of clinical responsibility, including liaison with college tutor and formal assessment of the placement.
    • To use specialist knowledge to inform service/policy developments and advise the service manager as appropriate.
    • To attend regular meetings with Clinical Leads

    FINANCIAL RESPONSIBILITIES
    • No budgetary responsibilities.

    TRAINING/EDUCATION RESPONSIBILITIES
    • Be an active member of in-service training, multi-disciplinary team working, and peer review.
    • To develop/maintain own competency to practice through Continuing Professional Development (CPD) activities, reading, clinical excellence networks (CENs) and external training courses.

    RESEARCH AND DEVELOPMENT
    • To carry out, as directed, the collection of data for use in audit and research projects within the service and /or trust.

    • Degree in Speech and Language Therapy or equivalent.
    • Registration with HCPC Post Graduate RCSLT accredited dysphagia course or equivalent.

    • Attendance at relevant Clinical Excellence Networks .
