Are you a Mental Health Nurse (RMN), Occupational Therapist or Social Worker looking for a new opportunity? We welcome you to apply for a Community Practitioner role within our new Community Mental Health Service.

We are seeking to recruit to North Shropshire Mental Health Team. We are undergoing Community Mental Health Transformation and are very excited to make an addition to our team with this role.

As an experienced Mental Health Nurse, Occupational Therapist or Social Worker, you will join a team where we work with the service user at the centre of what we do. We work in close partnership with service users and their carers during their episode of care. We offer assessments and interventions from a mutli-disciplinary team perspective.

Main duties of the job

  • Working in an identified community mental health team which is developing through transformation
  • Delivering a high-quality clinical service to service users and their families
  • Managing a complex caseload of service users and working with a range of clinical diagnoses
  • Responsible for clinical and caseload supervision of team members where appropriate (including student practitioners)
  • Operating as 'key worker' for a defined case load of service users
  • Delivering therapeutic interventions, in accordance with an agreed care plan, to an identified group of people experiencing mental health related difficulties

Job description Job responsibilities

You will be responsible for undertaking clinical assessments to establish the persons needs, goals, risks and strengths to generate a formulation-based care plan agreed with all involved.

You will be responsible for delivering therapeutic interventions, in accordance with an agreed care plan, to an identified group of people experiencing mental health related difficulties. You will be expected to support families and carers throughout this process, where appropriate.

You be expected to be available for consultation, support and clinical supervision to other health professionals / student practitioners working within the Community Mental Health Team.

Working Monday to Friday 9am 5pm and a valid driving license and car is required.

Person Specification Qualifications Essential
  • Registered mental health professional Eg. Nurse or Occupational Therapist or Social Worker
  • Post registration course/s at degree or diploma level
Desirable
  • Psychological interventions qualification (CBT, EMDR, CAT, DBT, Family Therapy)
  • Leadership & development qualification
Experience Essential
  • Evidence of extensive post registration experience of working with adults experiencing mental health related difficulties
  • Experience of assessing, planning, co-ordinating and managing people with mental health problems in a community setting
Desirable
  • Experience of leading CPA process
  • Experience of working in community setting and making decisions autonomously
