An exciting opportunity has arisen in Wigan Later Life and Memory Community Services.

We are looking for committed and compassionate Band 6 Nurse Practitioners to join our Later Life and Memory Services (LLAMS) in Wigan.

You will have a sound clinical knowledge of mental health assessment with both functional and organic problems, an understanding of concomitant physical health needs of older people with mental health problems and a sound knowledge of diagnosis and treatment of dementia. You should have a good knowledge of local services where clients can be signposted.

You will need to be registered with the Nursing and Midwifery Council, and have a proven track record of Continuous Professional Development.

The role will involve regular travel across multiple sites in the North West Boroughs Healthcare NHS Foundation Trust catchment area. Therefore, the post holder must be able to meet the travel requirements of this post. Please indicate on your application how you meet this requirement.

This post is subject to an enhanced check with adults barred list . Disclosure and Barring Service (DBS) Check. Successful candidates will either have a valid DBS check with the Trust or will be required to pay for the cost of the DBS Disclosure.

Main duties of the job

The post holder will support the Clinical Team Manager in carrying continuing professional and managerial responsibility for their clinical area. They will also contribute to maintenance of the overall standards, which will include assessment of need and delivery of care within the context of Effective Care Coordination. The post holder will work within an integrated team to ensure continuity of care for service users on their journey through the Memory Service. The post holder will work from within the community environment but be expected to be flexible in terms of delivering care across the borough as required
