Health and Wellbeing Care Co-ordinator, Band 6

Our Kent Prisons healthcare services are underpinned using our 'health and wellbeing' model. It has evolved over the past 5 years, where it was originally implemented at the Greenwich cluster. HMP Belmarsh in 2017 which was a first for NHSE (London Region) and a model which other services built on, for us followed by HMP Isis 2018 and HMP Wandsworth 2019.

There are core principles, however, how these are achieved are bespoke to the local healthcare and prison team during the model implementation:

• Patient focused with health promotion at the heart of our care
• Designing services which meet the unique needs of the prison
• The right staff, right place, right time ensuring experienced professionals are on hand to lead and support patients when needed
• A no exclusions model that ensures everyone with mental health needs is managed within the step care model
• Being highly supportive of clinical and prison staff with training, support enabling the team to work smarter and as part of an effective multi-disciplinary team

The six prisons are located across three estates, Isle of Sheppey, Rochester and Maidstone, with the Isle of Sheppey prisons being with a few minutes' walk from each other and East Sutton Park approximately 7 miles from Maidstone.

Main duties of the job

To manage, lead and co-ordinate the implementation of the health and wellbeing model within the Kent of Prisons. (model attached)

To provide and develop a specialised programme of care, or care packages in the Kent Prison Cluster that will address the prisoner's presenting vulnerabilities:

• Primary care needs and long-term conditions
• Mental health needs
• Substance misuse

To provide safe and effective care pathways for all offenders identified through the single point of assessment as needing care co-ordination.

Working alongside the responsible prison clinical team, the post-holder will arrange and organise meetings that will support the health and wellbeing model. These meetings include multidisciplinary meetings, case conferences, referral/discharges, and monthly monitoring meetings

Job description Job responsibilities

To case manage in conjunction with other practitioners offenders identified as appropriate. This includes developing a specialised programme of care, or care packages that will address the prisoners presenting vulnerabilities:

• Primary care needs and Long-term conditions
• Mental health needs
• Substance misuse

Be fully conversant with Trust policies, CPA, and the Mental Health Act, Primary Care Services, Substances Misuse Services (Psychosocial and Clinical Services) and all other services.

Provision of high-quality assessments and reports for example, when planning discharging and releasing of offenders into the community. Reports which include, for example a successful move from dependence on drugs /alcohol to recovery, the ability to access suitable accommodation, improvements in mental and physical health, ability to access education and community-based activities.

Delivering evidence-based protocols for the safe and secure care co-ordination of offenders whilst in custody.

The post holder will be expected to teach health professionals and prison staff on the health and wellbeing model, single point of assessment and care co-ordination process.

Act as the first line of arbitration for disputed care co-ordinated patients. This will involve identifying alternative solutions within the healthcare service.

Report all disputes and delays to the Head of Healthcare in a timely fashion.

Ensure that appropriate systems are in place, which enable and facilitate the effective care coordination and continuity of care.

Develop and maintain effective channels of communication, professional links and interfaces with NHS and independent providers, statutory agencies, the third sector and the Prison Service.

Working closely with these agencies, provide person centred and integrated packages of care, and co-ordinate activities to achieve joint working and integrated care arrangements.

Actively promote the development of multi-disciplinary clinical practice, embedding as appropriate current research and guidance into daily practice.

Support senior staff in the development of plans for the achievement of clinical excellence and demonstrate high standards of clinical practice to the team.

As required attend case conferences, clinical reviews, and CPAs.

As required attend and/or chair other meetings and forums as directed.

Ensure that all relevant clinical information is documented accurately and in a timely fashion onto SystmOne.

As required to provide supervision to junior staff, students and provide training within the service.

Practice within the NMC Nurses Code of Professional Practice and Conduct at all times.

Comprehensive knowledge of legislative procedures governing care and treatment of mentally disordered offenders.

Comprehensive knowledge of local and national policies in relation to patient confidentiality, Caldicott principles, Child and Adult safeguarding procedures and Vulnerable Adults Policy

Person Specification Education/Qualifications Essential

• o3 years post registration experience

• oRegistered RGN/RMN
• oCurrent NMC registration.
• oENB 998 or equivalent
• oEducated to MSc, BSc or degree level or equivalent experience
