We are looking for an enthusiastic RMN to join HEART (Home Engagement and Rehabilitation Team), GMMH's new and developing community rehabilitation team as a care co-ordinator.HEART care co-ordinators have small caseloads of up to 10 service users with complex needs. We offer intensive rehabilitation and support in community settings to enable early discharge from hospital.

The team currently supports service users in Manchester and Salford, and the role involves travel across these areas. As the team develops and grows it is expected that we will also move into other areas of the GMMH footprint.

You will require experience of working in a mental health setting and awareness of delivery of community mental health care.

The team has extended hours, offering our service users enhanced support from 8am - 8pm Monday - Friday . We are willing to consider applications from individuals who can work either on a rota basis over 7 days, or more traditional office hours.

Main duties of the job

The post holder will be responsible for the care co-ordination of a small caseload of service users. This will involve working creatively within a multi disciplinary team to support service users to identify their own recovery goals and co-ordinate timely and appropriate interventions to help them to achieve these and live as independently as possible. This includes respecting individuality, values, culture and religious diversity of service users and colleagues and thus contributing to the provision of a service sensitive to these needs.

The post holder will maintain a recovery focus and will demonstrate skill and sensitivity in the assessment of vulnerable service users with complex needs.

The post holder will also be responsible for the organisation and supervision of recovery workers within the team. They will also be responsible for and responding to crises where necessary, delegating tasks to colleagues as necessary. The post holder will play a key role in the development of the service, by assisting the leadership team in day-to-day operations, organising, leading, teaching, supervising and supporting junior colleagues.
