An exciting opportunity to work in partnership with Primary Care has arisen for two full time 37.5 hours, DMHLP Band 7 Nurse/OT Posts.

The team consists of 3 clinicians who provide a citywide service to the Integrated Neighbourhood Teams (covering 4-5 teams each). The successful candidates will also be integrated into the SSE Aire Court/ WNW St Marys Hospital, CMHT/MAS Teams.

The Neighbourhood Teams are run by Leeds Community Healthcare NHS Trusts with the aim of developing closer working between adult health and social care services across the city. The core team, including community matrons, district nurses, therapists, social workers and a neighbourhood clinical practitioner, all work together to provide care and support. Each team is clustered around GP practices.

Main duties of the job

The purpose of this role is to support primary and community care services and facilitate the development of the workforce, to meet complex needs arising from dementia and co-morbidities; including depression, anxiety, and delirium, linked to physical health long-term conditions and acute illness. The role is intended to provide team members with access to an expert practitioner who can provide support, education, and training to primary locality services. They will demonstrate a high level of practice experience within Mental Health Services.

The role will require you to build effective working relationships and networks with various stakeholders. You will also have the opportunity to be involved with service development both within LYPFT and the wider health and social care system.

Job responsibilities

The successful post holder will be employed by LYPFT and will be co-located within the Integrated Neighbourhood Teams. This post will be involved in the ongoing Community Mental Health Transformation Programme, a partnership of NHS organisations, Leeds City Council, the third sector and service users/people with lived experience, coming together to transform how primary and community mental health services are currently organised and delivered for adults and older people with ongoing and complex mental health needs.

You will be encouraged to have access to regular supervisions both management and clinical and to maintain your own professional development.

It is essential that you have a full UK driving licence and use of a car.
