As part of the implementation of Welsh Government Children and Young People's Continuing Care Guidance and local policies the post holder will be responsible for ensuring the assessment and review of individuals with learning disability needs currently funded locally and in out of area placements. The post holder will ensure that the relevant policy approach is implemented to complete individual, service and provider profiles, gather data on placements, including needs and risk factors, to inform local service commissioning, planning and governance frameworks.

This post holder will play a pivotal role in collaborative working across the health, education and social care interface, to support the development of integrated community services. The post will involve extensive cross boundary working across the health, local authority, voluntary and independent sectors.

Main duties of the job

Ensure optimum standard of assessment is delivered at all times by all levels of staff within the team. Adopt a proactive approach and encourage all members to aim towards and achieve high levels of competence, skill and professional development.

Prepare clear concise reports, and use I.T. systems to maintain the service user and All Wales database in line with Caldicott principles.

Work collaboratively with partners in Health, Education and Social Care to problem solve and implement change, building up a robust network to support sharing and replication of best practice.

Possess an extensive understanding of the Welsh Government Guidance on Children and Young People's Continuing Care, be familiar with all relevant policy principles e.g. Decision Support Tool, criterion for eligibility, and maintain a knowledge base and skills in current healthcare issues and guidelines.

The role is an exciting opportunity to develop a new service for SBUHB working alongside a dynamic community children team. The role will involve assessment of referral of children with learning disabilities and having the skills and knowledge to support them and their families to access the care needs they require to allow them to develop and reach their full potential.

Provide leadership and support to professionals working in Childrens community nursing or the inpatient ward areas when there are children requiring LD input.

  • oKnowledge and experience of working with individuals who have complex ongoing care needs associated with Learning Disabilities
