Job Details

The Julian Hospital is an older adult mental health acute admissions in-patient facility, which comprises five wards on the Julian Hospital site in Norwich and a satellite ward at Carlton Court, Lowestoft.

As a Band 6 Physical Health Practitioner you will be responsible for ensuring that all in-patients have the appropriate health checks, be involved in the developing person-centred physical health care plans, health promotion and education of healthy lifestyle choices. You will work closely with the Multi-Disciplinary Team to advise about physical health interventions and any recommendations, attending Clinical Reviews to discuss cases, educating staff in physical health interventions and work autonomously to devise training packages to deliver to staff to enhance their skills and knowledge in physical health.

You will need to be enthusiastic and conscientious and do everything you can to provide the best care to our patients. We are looking for someone who displays compassion, empathy and sensitivity in your approach to people who may be distressed or have a challenging manner.

Main duties of the job

Admission physical health review, Bloods, ECGs, Skin checks, observations, health screening - including MUST, Waterlow, VTE and Falls review.

Receiving referrals to review patients for differing physical health concerns. Including - diabetic care plan reviews, complex wound care, catheter care, pressure area care reviews, escalation of NEWS and palliative/end of life support.

Training and teaching all staff on a variety of physical health subjects. This includes signing off competencies for health skills including ECGs, phlebotomy etc.

Creating, developing, and delivering in-house bite-size training sessions.

Liaising with external healthcare company representatives to delivery training on specialised subjects.

Liaising with community healthcare professionals for expert advice, such as diabetic nurse specialist, Parkinson's and palliative care specialists and the district nurse teams.

Supporting student nurses in learning, training and supervising practice.

Phlebotomy mentor, supporting students in practical learning and skills according to the national phlebotomy guidelines and protocols.

Auditing, collating, collecting and reviewing trust audits for physical health screening and compliance.

Supporting with dietetic advise

Smoking cessation adviser

Support the ECT clinic to recover patients pre and post procedure.

Involved in AHP group directives and discussions

Adhering to Trust guidelines and Trust protocols.

  • Registered Health Care Professional
  • Experience of post qualification of 2 years Experience of developing, delivering and evaluating training particularly around behaviour/ lifestyle change Experience of delivering health promotion/ public health interventions Long term conditions, wound care physiological monitoring to prevent clinical deterioration Experience in creating, implementing and reviewing audit data.
  • Able to teach ECG application, phlebotomy, physiological monitoring procedures Must be able to use computer and perform basic word processing
  • Knowledge of public health / and NHS improvement initiatives Well person screening, physiological monitoring tests and selection of interventions or referral options for wound care, respiratory illness such as COPD Diabetes, sexual health and family planning.

Desirable
  • Degree in relevant health care subject Teaching qualification Post registration qualification in, health promotion including smoking cessation, sexual health and well person screening Leadership and management qualification
