We are recruiting a Lead Primary Care Network (PCN) Mental Health Practitioner to join our team in Fakenham and Wells. This role is open to a Nurse, Occupational Therapist or Social Worker.

This role will be covering 1 or 2 practices.

Now is a really exciting time to join us as we are expanding our team and launching our Primary Care Network across the whole of Norfolk and Waveney. Primary Care Network (PCN) Service for Mental Health has been developed to provide access to mental health services in Primary Care as part of this integrated health and care system, which brings care closer to home for our population.

Main duties of the job

As a Senior PCN practitioner, you will be responsible for the management of day-to-day operational and clinical issues arising from within your PCN. You will line manage the PCN practitioners as part of a multidisciplinary team providing advice, support, and treatment for members of our community experiencing problems with their mental health and wellbeing. You will collaboratively engage with the GP practices within the PCN and the PCN Practitioners to agree priorities for each local population.

We are looking to recruit Mental Health Nurses, Registered Nurses, Occupational Therapists and Social Workers with an interest and experience in a mental health setting, to undertake the planning and delivery of direct patient care.

It is an autonomous, solution focused role which noes not hold a case load. It also offers a great work life balance, as the hours are in line with the extended hours of service of GP practices.

These roles, although new have been piloted for the past year in our Early Adopter sites, so the workload is defined, manageable and appropriate to allow for patient centred care and other activities.

Essential
  • Nursing degree or equivalent or Dip OT Dip OT, Dip SW, RN, Clinical Psychology Grade A, or equivalent.
  • Ongoing professional development working towards degree level.
  • Higher degree or post graduate certificate in area of clinical / therapeutic practice Post basic teaching qualification

Experience
Essential
  • Able to demonstrate an appropriate level of knowledge of mental health practice from previous experience.
  • Experience in the delivery of short term, focussed evidence-based interventions.
  • Experience in providing leadership in a mental health care environment.
