To take a role in providing the successful and effective delivery of physical health assessment and treatment in Clayfields Local Authority Secure Children's Home

To work as a physical health nurse within a multi-disciplinary team in a secure Childrens home setting. Providing care to young people with complex needs, from the point of entry into Clayfields House to re-entry into the community so that the health and social functioning of young people is improved.

To provide education and supervision to staff members and other professionals ie education staff, care staff.

To support local Band 6 physical health nurse, psychiatrist and GP's in delivery of health clinics.

Main duties of the job

  • Judgements involve a range of facts or situations, which require analysis, interpretation and comparison of a range of options.
  • Assessment of young person's condition, recognising deterioration, evaluate outcome of an intervention and feedback into service change.
  • Judgements on problems requiring investigation/analysis including assessment of young person's condition ie wound care.
  • To undertake individual young person's assessments, plan and implement effective treatment regimes in conjunction with agreed objectives between young person, yourself and other professionals.
  • Recognise own professional limitations and seek support when required.
  • Demonstrate the ability to carry out a comprehensive assessment of health, wellbeing and social care needs.
  • To modify the care package and where appropriate refer to external agencies in a timely manner.
  • An understanding of how medication, diet, physiological and external factors can impact on patient care.

Job responsibilities

The role is fast paced and challenging and requires the team to provide care to young people who have offended or are at risk. We provide successful and effective delivery of mental health and physical care to young people with complex mental health problems, from the point of entry into Clayfields House to re-entry into the community.

The candidate must be a currently registered physical healthcare nurse, have experience working with young people, working and liaising with different agencies, and have relevant experience and skills of assessment and delivering evidence based interventions.

The role will has a flexible shift pattern working and being part of an on-call rota only for new admissions into the centre.

  • Holds UK driving license

Qualifications

  • Professional qualification, registered nurse (general) to degree level or equivalent.
  • Maintains current registration with the NMC

  • Demonstrate continued professional development to maintain professional registration

  • Willingness to undertake new clinical practices, education and training that will enhance service delivery

Experience
Essential
  • Demonstrates satisfactory clinical expertise for the banding

Desirable
  • Specialist experience working with young people
  • Experience working in secure services

Knowledge

  • Knowledge and understanding of current legislation and understanding of clinical risk assessments
