An excellent opportunity has arisen for an experienced Band 6 Mental Health Liaison Practitioner in Milton Keynes. This is an exciting opportunity to work in an already established multi-disciplinary team with a dedicated workforce. The Mental Health Liaison Team in Milton Keynes operates under a CORE 24 Service Model.

You will need to have substantive post registration experience as an RMN or Occupational Therapist or Social Worker as well as some relevant knowledge and experience relating to discharge planning and Adult Social Care.

We can offer some flexible working and the opportunity to earn financial enhancements through night shifts/weekend and bank holiday working.

Main duties of the job

The post holder will be expected to work as part of a multidisciplinary team and be able to conduct comprehensive biopsychosocial assessments for patients that present in the Emergency Department and the general wards at Milton Keynes University Hospital (MKUH) with mental health crisis and/or mental health problems. This is an exciting opportunity for an experienced nurse or OT or Social Worker to develop and support a strong working relationship between Milton Keynes Community Health Services and Milton Keynes University Hospital. The post holder will be responsible for facilitating the most appropriate treatment pathways for individuals currently in a hospital ward or Emergency Department who are believed to be suffering from a mental illness or are in mental health crisis.

The post requires an individual who is motivated, enthusiastic and who has exceptional risk assessment and decision-making skills with an active interest in discharge and pathway planning for both adults and the older person. There are also opportunities for the post holder to participate in teaching and training sessions for staff at MKUH.

You will work as part of a multi-disciplinary team with the opportunity of developing links with other professionals in a wide variety of settings. We offer training opportunities, regular supervision and performance reviews and post graduate education is encouraged.

Job description Job responsibilities

The Mental Health Liaison Practitioner will work as part of the Liaison service hosted by CNWL NHS Foundation Trust. They will act as a first line of contact providing assessment, advice and liaison within Milton Keynes University Hospital Foundation Trust, with the support of the Mental Health Assessment Service.

The post holder will support people presenting at A&E and on MKUHFT wards, with mental health problems or suspected underlying mental health needs by means of assessment and facilitation of the most appropriate treatment pathway.

Person Specification Qualifications Essential
  • Professional Qualification at Diploma or Degree level in Mental Health nursing or other professionally qualified health professional. Valid registration essential.
Experience Essential
  • Extensive experience working in one or more areas of psychiatry; General Adult, Older Adult, Addictions, Perinatal, Forensic, Liaison, Social and Community.
  • Excellent knowledge of MHA 2007, the MHA Bill and MCA Varied clinical approaches. Client advocate.
  • Experience working in acute crisis with people who deliberately self-harm
  • Experience in assessing acute crisis and risk of individuals and putting robust safety plans in place.
Knowledge Essential
  • Excellent knowledge of MHA and MCA
  • Sound knowledge of principles of recovery
  • Excellent knowledge & experience of assessment and risk assessment tools
  • Understands & promotes equality, diversity and human rights in accordance with legislation, policy and procedures relating to recruitment (KSF 6.2)*
  • Understanding of confidentiality in the workplace
