We are looking for an experienced, motivated and enthusiastic Lead Therapist for Integrated Services in Central Monmouthshire. The team operates out of Monnow Vale Health and Social Care Facility in Monmouth and the Wellbeing Centre in Usk.

As Lead Therapist, you will provide clinical and professional leadership to therapists in the Integrated Team, as well as being an integral part of the wider multidisciplinary service comprising District Nursing, Social Work, Memory Service, Community Ward, Day Services and Direct Care (including Reablement). You will be expected to develop and maintain excellent working relationships with a range of professionals and community organisations, in order to support individuals living in the local communities, in line with the Clinical Futures programme and place-based care.

You will have day-to-day oversight of therapeutic interventions for individuals living in the community, inpatients on the 19 bedded community ward at Monnow Vale and for individuals residing within the Secure Estate.

You will need to be a highly skilled therapist who possesses direct community experience and will be expected to carry a defined caseload of individuals requiring therapy support. You will be conversant with relevant legislation that informs the development of community care services locally and will be involved with any future proposed changes to services in Monmouthshire.

Main duties of the job

The post holder will be responsible for planning and delivering the integrated therapy service in Monnow Vale Health and Social Care Facility in close collaboration with the Central Monmouthshire Integrated Services Manager and the Operational Community Team Manager. They will be responsible for the co-ordination and evaluation of service delivery ensuring effective management of human and other resources. The post holder will provide clinical and professional leadership to the therapists in the Central Monmouthshire integrated Service. Particular emphasis will be on maintaining the patient flow through the hospital and delivering the objectives of the Monmouthshire integrated Service. In addition to managing a varied caseload of patients they will have a line management responsibility for the therapists and will provide input to the planning, development and evaluation of the integrated therapy services for Monnow Vale Health & Social Care Facility.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
