An exciting opportunity has become available for a Band 6 Community Learning Disability Nurse to join a busy team based in the Swansea area.
The post holder will have an understanding of the knowledge and skills of the professional activities within a Community Support Team within the field of Learning Disabilities. They will be responsible for managing a clinical caseload, and will liaise with both Social & Healthcare professionals to meet client need.
Contribute to the development, maintenance and evaluation of systems and structures, which promote the rights, responsibilities and diversities of people with a learning disability, through effective communication.
The post holder will promote the health and well-being of clients by providing specialist nursing assessment and advice and training to clients, carers and other professionals.

Main duties of the job

- Evidence of post registration educated to degree level
- Evidence of experience working within MDT
- Experience and, or knowledge of risk management
- Experience of working with families
- Experience of working with individuals with complex health needs and challenging behaviour
