Assertive Outreach services are provided for people with a primary diagnosis of serious mental disorder often co-existing with substance misuse. It is for those patients who are difficult to engage, have frequently failed with routine treatment programmes, exhibit poor social functioning, are often excluded from many aspects of daily life and are deemed to be a risk to the local community, themselves, carers, families or significant others.

The Assertive Outreach Team aims to improve engagement of Service Users, increase stability in their lives and work with their Carers/Family members, reducing hospital admissions and their length of stay when hospitalisation is required. The team will also aim to improve the service users' quality of life, social functioning, maintain and manage mental health and wellbeing, access to services. :

Main duties of the job

The role is professionally supported by the Lead Occupational Therapist for Mental Health Rehabilitation.

The Assertive Outreach Team West covers Dorset county excluding Bournemouth, Poole and Christchurch therefore the ability to drive your own vehicle is essential.

  • . Occupational Therapist diploma or degree
  • State Registration with HCPC
  • . Membership of the relevant Professional Body
  • Evidence of on-going professional development in an up to date portfolio
  • . Ability to achieve a pass mark (75%) in the ICT Basic Skills test
  • . Ability to work autonomously in the community making appropriate clinical decisions and managing risk without constant and direct supervision. Able to identify when issues require joint working and escalation into the wider multi-disciplinary team.
  • . Knowledge of lone working policy requirements and the risk assessment skills required to promote safe lone working

  • Significant post registration demonstrable experience within mental health services, working with people with severe and enduring mental illness and complex needs

  • A clear understanding of the function of a rehabilitation service and a demonstrable knowledge of therapeutic interventions used within the rehabilitation service
  • . The ability to provide clinical / management supervision
  • . Understanding of therapy outcome measures
  • . Ability to work autonomously and as part of a team; recognising own limitations & when to ask for support and advice.
  • . Comprehensive knowledge and application of therapy assessments, including MOHO assessments and interventions relevant to the client group
