At Home Treatment, we pride ourselves in providing outstanding care to patients across Herefordshire and Worcestershire and are looking for individuals who have clinical skills that encompass urgent mental health and home treatment interventions and are passionate about providing a service to meet the needs of our patients.

Home Treatment is integral to our urgent care pathway, providing a specialist service offering comprehensive assessment, treatment and care of service users. You will work collaboratively with service users, carers and professionals, offering interventions as an alternative to hospital admission.

We support early discharge from inpatient wards, through intensive but time limited intervention in the community. Home Treatment also supports primary care through screening/assessment of GP referrals. Positive risk management is vital in this role.

Main duties of the job

To work as a member of the Home Treatment Team, providing intensive home treatment to patients and support for their families in accordance with Trust and Home Treatment Team Policies and Procedures.

To manage a defined caseload of patients experiencing an acute mental health episode, using evidence based, client centred, hope and recovery, health promotion and relapse prevention orientated principles and practice.

To assist in identifying, developing and providing a multidisciplinary approach within the service in accordance with the needs of service users and the requirements of the service.

To utilise a range of therapeutic interventions for adults with acute mental illness.

Working with service users in their own homes to promote maximum independence whilst experiencing mental health crisis.

To provide effective supervision, support and guidance for non-registered workers and any work delegated to them. To participate in planning of own clinical supervision.

You will actively participate in the development of the service and participate in the overall development of the service ensuring the principles of Clinical Governance underpin all developments.

Use of a car for work (due to the rural geography of Worcestershire) is essential to this post.

Job description Job responsibilities

The role includes remote and independent working to complete urgent mental health assessments/reviews in community settings as well as home treatment for adults of working age with acute mental health needs often at times of crisis.

To improve the lives of people with mental ill health and /or learning disabilities by minimising the impact of their condition through the delivery of excellent services to promote recovery and wellbeing.

To facilitate others in the team to develop competence by providing leadership, day to day supervision, clinical advice and clinical supervision to junior clinicians and students as appropriate.

Undertakes risk assessments/risk management in accordance with the Trusts Clinical Assessment and Management of Risk to Self and Others for Patients in Mental Health Services Policy, devising and implementing actions and intervention plans which take the risk formulation fully into account.

Takes steps to obtain patient consent to care and treatment in accordance with the Mental Capacity Act, ensuring that patients who lack mental capacity remain at the centre of decision making and are fully safeguarded.

The ability to develop, implement and evaluate intervention plans as part of the multi-disciplinary team, with the involvement of the patient and where appropriate their carer.

To contribute to the planning and implementation of induction, training and education of students and new staff.

To participate in the preceptorship programme during the first post registration year in the NHS.

A willingness to undertake practice placement educators course and participate in student education.

To assist the senior staff by identifying personal and professionals training needs in order to appropriately plan for future training and development.
