If you're a Mental Health Nurse, Occupational Therapist, Social Worker, or registered professional working within Mental Health and want to work with and across a variety of GP surgeries on the beautiful North Norfolk Coast - this could be the ideal role for you!

Our Coastal service covers the area including Hunstanton, Brancaster, Marham, Swaffham and all of the beautiful villages in between.

As PCN Mental Health Practitioner, you'll play an incredibly important role in transforming community mental health services. We're working collaboratively with the Integrated Care System to offer 'Place Based Care' - ensuring communities receive fast and effective treatment, in the familiar surroundings of their GP surgery.

Main duties of the job

You'll work alongside our lead, Krisa, providing support, treatment, advice, education and problem-solving approaches to members of our community experiencing challenges with their mental health. You'll be providing a dedicated service that makes sure needs are met in a seamless environment.

I was attracted to working in the Primary Care network after hearing about the transformation work that is happening across the local healthcare system.Since joining the team I have found working with service users alongside their GP's to be an extremely rewarding venture and offers a new perspective for my skills as a mental health professional.I have found being able to help influence the decision making for service users and providing interventions at an earlier point in their care pathway an exciting opportunity. The challenges of building relationships within a new team in a different arena has been invaluable in terms of my development which ultimately benefits those in receipt of the care I'm providing.

Although this is a full-time opportunity, we're flexible and are happy to discuss different working patterns, provided these are within the GP practice working hours.

About us

Primary Care Networks (PCNs) are groups of GP practices who work together in a local geographical area - in this case, our Coastal region which covers a beautiful, diverse area. You will collaboratively engage with the GP practices in your area and will be an extension of their team.

As a Mental Health PCN practitioner, you will be responsible for the day-to-day operational and clinical issues. You'll work at a quick pace, autonomously managing your own clinics and will provide an intervention led approach, acting as an incredibly important first point of contact for people who need care and support the most.

The ideal candidate will be conscientious, kind and compassionate - you'll be able to work well independently but will also have the support of the wider team. . Driven by the needs of those who use our service,you will need to have good organisational skills and experience in a mental health setting. You will be an autonomous practitioner, making clinical judgements, identifying alternative courses of action, managing, and providing high-quality care.

Person Specification
  • Nursing degree or equivalent or Dip OT Dip OT, Dip SW, RN, Clinical Psychology Grade A, or equivalent.
  • Ongoing professional development working towards degree level.
  • Higher degree or post graduate certificate in area of clinical / therapeutic practice Post basic teaching qualification

  • Further qualification in mental health work. ILM certificate in management or equivalent

  • Able to demonstrate an appropriate level of knowledge of mental health practice from previous experience.
  • Experience in the delivery of short term, focused evidence-based interventions.
  • Experience of working with mental health service users in a statuary or voluntary capacity.
  • Experience in providing leadership in a mental health care environment.
  • Experience of working within a multi-disciplinary team.
  • Evidence of leading service development within a mental health care setting.
  • Experience with risk assessment and ongoing management.

  • Understanding of mental health issues in a primary care setting

  • Excellent communication skills Competent assessment skills
  • Effective analytical and decision-making skills Care planning skills.
  • Competent and effective treatment skills. Able to effectively prioritise own workload.

Norfolk and Suffolk NHS Foundation Trust is the main provider of specialist mental health services for the counties of Norfolk and Suffolk, serving a population of 1.6 million people. We are a large organisation covering a wide geographical area divided into ‘Care Groups'. There are also Care Groups supporting children, family and young people’s services, and specialist services, including Forensic mental health services.
