Exciting opportunity for a full time Band 5 Community Nurse to join a busy Community team.
To provide clinical expertise, knowledge and skills in the promotion and maintenance of optimum health for adults with learning disabilities and complex health needs.
To operate as a practitioner within a learning disability team providing person centred health care.
To improve knowledge and awareness of issues impacting on adults with learning disabilities, and appropriate action to support clients.
The delivery of clinical interventions and treatment to meet the clients assessed health care needs.
Liaise effectively with community and specialist organisations (including acute, local authorities and voluntary organisations).
Responsibility for the implementation of quality assurance systems within a Clinical Governance framework.
Main duties of the job
The Neath CLDT is looking for an enthusiastic Band 5 Community Learning Disability Nurse to join their busy team.
The CLDT covers a diverse population and is passionate about reducing health inequalities and improving health.
The successful applicant will be part of the current plans to modernise community services.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

- N.M.C. Registered
- Experience of working within a multi-disciplinary/multi agency context.
- Good communication skills both written and oral.
- Ability to demonstrate a good knowledge of professional boundaries, and the ability to make decisions in accordance with your professional Code of Conduct.
