Main duties of the job

We are recruiting four nurses for the borough for Hammersmith & Fulham, Ealing and Hounslow. We want to hear from you if you are dedicated to forward-thinking with clinical skills and experience in working therapeutically with older people with mental health difficulties and physical healthcare issues.

This post is ideal for experienced B5 who wants to progress to a senior position or less experienced B5 who can be considered for a Developmental B5 to B6 Post.

The Developmental Post means that they will be employed as B5 and will be promoted to a B6 post after all competencies are met, ideally within 6-12 months.

We ensure as an employer that the required support for the competencies will be on hand. You will be supported throughout your clinical journey by senior clinicians within the multidisciplinary teams.

Job description Job responsibilities

The post holder will be based in one of the community teams in Hammersmith and Fulham or Ealing and they will be undertaking home visits, meetings at the team base and other community sites.

The post holder will work under the supervision of a B7 Senior Practitioner. The post holder will carry a defined caseload of patients. They will utilise their clinical knowledge and skills in the assessment and care of Older Adults with functional mental health problems in the community. They will work within a multi-disciplinary team to undertake the facilitation of groups and individual therapeutic activities.

The post holder will be responsible for the supervision of junior staff and students as well as contributing to departmental and Trust improvement projects.

Person Specification Qualifications Essential

• Professional qualification in mental health - Mental Health Nursing (RMN) with current NMC registration

Desirable

• Educated to degree or Diploma level with other courses relevant to primary care mental health services e.g. CBT, Brief Solution Focussed Therapy etc.
• Evidence of recent and on-going relevant further professional development

Experience Essential

• Working with people with mental health and physical health problems Working within a multidisciplinary team environment
• Experience of supervising junior staff within own and/or other disciplines

Desirable

• Experience of teaching/training in a formal or informal setting
• Experience of working in a primary care setting/ social care/ voluntary sector
• Lived experience of mental health issues/caring for Older person with mental Health difficulties
• Audit and research experience

Knowledge Essential

• Knowledge of counselling skills/cognitive behavioural skills, psychosocial interventions
• Knowledge of recovery approaches as used with those with mental health conditions
• Interpret research and apply to practice
• knowledge of mental health conditions in relation to Older people as well as their relationship to physical disorders

Desirable

• Current developments in nursing
• Dementia awareness
• Knowledge of long term physical health conditions

• Ability to frequently work under pressure and maintain intense concentration, particularly in unpredictable situations where there will be exposure to highly distressed patients, who may exhibit challenging behaviours .
