To provide clinical leadership, support and supervision to clinical staff. Tosupport an identified team of practitioners with advice and guidance on themanagement of complex social care cases.o To work as an integral part of the CMHT which will include carrying a smallcomplex caseload.o To participate in the extended working hours as required by the needs of theservice- Maintain consistency and quality of care for all service usersunder the care of the CMHT.- Support safe, efficient and effective high quality transitions in and outof the CMHT- Ensure the team works in a collaborative manner with otherPCFT services, primary care, the local authority and otherpartners- Support the team in delivering recovery focused mental healthcareo To ensure that clinical standards are, monitored and evaluated, andensure both they and the team provide care of the highest quality topatientso To liaise closely with social care leads, housing associations, IPSworkers and other agencies to promote social support and aidrecovery and promote this way of working within the teamo To liaise closely with, general practitioners, to ensure clinicalpathways are supportive of patient flow from CMHT's to PrimaryCare and vice versa

Job description Job responsibilities

To be responsible for the development and maintenance of effectiveclinical pathways between CMHT and other services. To take a lead role in the provision of clinical leadership, positive problem solving andstructured clinical supervision to the team. To lead on and ensure that the team undertakes appropriate in-depthassessments and on-going intervention and undertake the duties andresponsibilities of a lead worker/care co-ordinator, maintaining an activelead worker caseload in line with policies and frameworks. To work collaboratively with medical staff/GPs and the Local authority to produceclinical management plans. To ensure the provision of the highest possible standard of care within theCMHT and ensure the team follow these principles. Ensure patients withinthe team have an up to date integrated care record, reflective of their riskand care needs in line with policy and procedures. To assist people to achieve maximum independence through the use of careprogrammes, which are culturally sensitive, individualized, safe andconsistent with accepted practice and ensure the team also follow theseprinciplesAFC 26/10/22 JME-210-22 Mental Health Social Care Clinical Lead Band 7 To participate and lead in mental health and social care education andprovide advice/support as necessary to patients and their carers on themaintenance of good health and prevention of relapse and ensure the teamalso follow these principles To receive formal managerial supervision from the Team Manager andclinical supervision from a suitable expert. To initiate, develop and implement evidence based clinical practice integrating socialcare requirements/duties. To lead on Safeguarding issues within the service To participate on policy implementation with TMBC. To participate on carer and service user involvement, working closely withother trust leads in rolling out carer/service user support forums.

  • Registered professional Nursing/Social Work/Occupational Therapist. Diploma or degree in a professional qualification Evidence of post registration additional training or experience to post graduate diploma level
Knowledge/Experience Essential
  • Significant experience of leadership and proven track record of managing groupsvidence of high levels of competence with written documentation Evidence of achieving high Personal Experience of mental health or experience through caring Proven track record of successful strategic thinking Experience of leading on audit and/or research activity related to clinical practice Application form/ interview/ references 2 standards regarding CPA documentation Evidence of building and maintaining effective relationships of staff and/or teams Successful management and control of substantial and complex resources Evidence of improving service quality Successful track record of prompting, leading and implementing change programmes that drive up standards and service performance History of holding responsibility and accountability for clinical decisions and empowering clinicians to make difficult clinical decisions Evidence of high levels of competence with written
