We are looking for an RMN who is highly motivated, innovative, enthusiastic and passionate about service user centered care to work within the Assertive Outreach Team based in Newport.Assertive Outreach is an approach to supporting recovery for adults living with serious mental health difficulties alongside complex social circumstances, needs and risks and who find it difficult or do not want to engage with structured care and treatment available in the Adult Community Mental Health Services (CMHT).

You will have knowledge and experience in the care of people with complex, severe and enduring mental health problems in a community setting. You will have opportunity to link with the wider service which includes a Crisis Resolution/Home Treatment Team, Primary Care Mental Health Support Services (PCMHSS), Community Mental Health Teamsand Inpatient Services. You will be committed to ensuring the delivery of the highest quality user-centered care and contribute to the development of mental health services in the Borough of Newport

The post is a permanent post with fulltime hours. Flexible working patterns will be available

Main duties of the job

The post holder is a senior member of the Assertive Outreach Team and will be responsible for the care coordination of a caseload of service users who are experiencing severe mental health difficulties. The post holder will take responsibility within their caseload for ensuring the assessment of health and social needs, formulation, planning, delivery and evaluation of care, support and treatment by the team, for both individuals and groups of service users including risk assessment and management, CTP, and care reviews. They will work collaboratively across a wide ranging multi-disciplinary services and agencies to provide effective delivery of health and social care, supporting a programme of recovery and social inclusion within the area.

The post holder may also supervise junior staff who are engaged with service users within their caseload.
