West Kent Primary Care Mental Health Service are currently undergoing a recruitment drive, including for this exciting post for an Adult Nurse, or Registered General Nurse with an interest in working with people with mental health difficulties, and a passion for promoting health and wellbeing. Patients with severe and enduring mental health problems suffer from more physical health problems and have a lower life expectancy that the general population, and the aim of this post is to undertake regular health checks of patients receiving treatment from the team, promoting health and wellbeing in addition to supporting your colleagues to improve physical health knowledge. With opportunities for both individual and group work, the post holder will undertake wellbeing and medication clinics. Main duties of the job
To work as a member of the Primary Care Mental Health Team, providing physical health nursing assessments, care and treatment for adults with serious mental illness within Primary Care in the community. This role also involves supporting and promoting physical well being and educating service users, carers and colleagues. An awareness and knowledge base of service provided by the Primary Care Mental Health Service for severe, enduring mental illness. Experience and knowledge in the management of long term conditions such as diabetes and heart disease Delivery of patient care to reflect and refer to Evidence Based Practice

Person Specification Qualifications Essential
- Qualifications: min of 3 years post qualifications; Current professional registration with appropriate body in healthcare or social work
- Leadership or management qualification:
- Experience: Significant experience working with people with mental ill health in a community setting
- Knowledge: Demonstrate knowledge to post-graduate level of evidence based practice in caring for patients in the designated field
- Communication Skills: Communicate complex and sensitive information effectively to patients, cares/ families and all members of the multidisciplinary team
