37.5 hours during Monday to Friday.

This role allows the possibility of hybrid working, meaning you will be required to commute to the stated base 2 or 3 days per week but allowing you to based within any commutable distance for the remainder of your week. We would need to discuss the suitability to fit this around the service at interview.

In the Redditch Neighbourhood Mental Health Team (NMHT) we strive to improve the mental health and wellbeing of our local communities.

We have incorporated a huddle into the daily functioning of the team which enables us to be focused on the recovery of the patient through a multidisciplinary approach, with expertise from doctors, nurses, occupational therapists, support workers and psychological therapies staff.

Main duties of the job

Will work as a key member and resource of the MDT core team.

Hold a blended caseload of patients with severe mental illness however your caseload will reflect your strong professional interest in supporting people with complex presentations related to severe and enduring mental illness and difficulties engaging with services. This post will have dedicated time to engage with local MAPA and MARAC processes and to provide specialist risk assessments for the locality team.

Supporting patients in an assertive outreach approach and identify any ongoing care needs, and assess and manage risk.

You will have experience of working with this user group in community settings. You will have a registered qualification in mental health nursing, occupational therapy or social work.

Ideally you will have experience in Solution Focused Therapy, CBT, Structured Case Management, and experience of using a specialist risk assessment such as HCR-20. We will provide this training, following a successful probationary period for the right candidate.

The ability to work flexibly as the need of the service demands is essential.

Attend weekly MDT meetings, participate in professionals meetings, signpost to adjunct services depending on patient need.

Act as a role model to colleagues, always seeking to maintain the highest standards of professionalism.

Use their initiative and take responsibility for themselves and the quality of their work and the service they provide to patients.

Work across all clinically appropriate patients, 17-years-old and over, to deliver evidence-based interventions.

Under the direction of Service Lead and as part of the Community Mental Health Service, provide prompt local assessment, triage, risk assessment, risk management, signposting, onward referral and evidence-based interventions for people presenting with mental health problems.

Provide a presence for consultation for all primary care and VCS staff.

Represent the team on specific projects and working groups as required.

Person Specification Qualifications Essential
  • Relevant professional qualification relating to Health or Social Care e.g. in social work, nursing, OT with current professional registration
  • RMN, BSc/DipSW, BSc OT
  • Recent evidence of continuing professional development
Knowledge Essential
  • Knowledge of principles of assessment of people with mental health problems
  • Knowledge of principles of risk assessment and management
  • Knowledge of Mental Health Act 1983 and Mental Capacity Act 2006 and other relevant legislation
Experience Essential
  • Experience of working in a Mental Health setting
  • Assessment of people presenting with mental health problems
