An opportunity has become available for a Senior Mental Health Practitioner to provide specialist mental health care as a valued member of the Primary Care Network (PCN). The post will be based within the Spalding PCN and It will include working with primary and secondary care and the neighbourhood teams to locally support the mental health transformation and improve the lives of people who access healthcare services.

Main duties of the job

Key elements of the role will include (but not be limited to):o Providing specialist clinical knowledge and interventions to service users in primary care setting. This will include working with the practice team to support people with highly complex mental health needs/serious mental illness.o Developing and implementing clinical processes that contributes towards the continuous improvement of mental health provision across the primary care network (multiple GP practices)o Providing best practice training, mentorship and guidance to mental health practitioners and other roles within the primary care/ neighbourhood MDTo Managing a caseload of highly complex/SMI presentations working closely with the locality secondary mental health service, to maintain personalised care plans which focus on what matters to the person and avoid escalation to.o To perform the duties of a non-medical prescriber, ensuring that appropriate medication is prescribed, monitored, and administered in line with current best practice and the Trust Medicines Management Policy.The post holder will be encouraged to contribute to service development, and there will be opportunities for CPD and clinical supervision as professional development is actively encouraged.

The post will be based within Spalding PCN covering Munro Medical Centre and Beechfield Medical Centre.

The successful candidate will be an experienced clinician with proven skills in working with people who have a range of highly complex mental health needs, demonstrating practice that is evidence-based and innovative. Demonstrating adaptability, excellent communication skills and self-motivation. In addition to undertaking mental health assessments, you will take on a caseload of highly complex service users to provide therapeutic interventions and coordinate their care. You will work in the context of local and national drivers providing advanced clinical care for identification of individual needs which essentially supports diagnosis and treatment options.

Ideal candidates will also be qualified to prescribe and support with medication decisions. Person Specification Qualifications Essential
  • Degree in mental health and social care or equivalent:
  • 1st level registration RMN
  • Evidence of specialised continued professional training (degree level) in clinical practice
  • Non-medical prescriber (or ability to complete within first 12 months in post)
  • Evidence of advanced clinical practice at post graduate level to masters or equivalent
  • Evidence of continuous professional development
Desirable
  • Management or leadership qualification
Experience Essential
  • Post- Registration experience of working with adults with mental health needs.
  • Experience of working autonomously within a senior position
  • Experience of managing staff and good leadership skills.
  • Experience of working across organisations
  • Sound knowledge of the national agenda for mental health.
  • Sound knowledge of clinical/ risk assessment and understanding of confidentiality.
  • Sound knowledge of the Mental Health Act.
Desirable
  • Experience of leading service change/development/transformation.
  • Experience of leading training sessions
  • Experience of neighbourhood working
Skills & Competences Essential
  • Highly developed clinical reasoning skills
  • Prioritisation and resource allocation, planning and organisational skills.
  • Able to communicate complex and at times highly sensitive information to a variety of stake holders
  • Good negotiation skills for conflict resolution
  • IT skills- email, word processing, excel, systems use
  • Analyse and interpret data and/ or information.
