City East Community Mental Health Team (CMHT)

We currently have a fantastic opportunity for an experienced Mental Health Nurse to apply for Band 6 Community Psychiatry Nurse (CPN) post in the City East CMHT.

The post holder will have a clinical case load and will be supported by the Multidisciplinary team in managing our client group.

The teams are dynamic and constantly evolving to offer the best support and treatment to the patients that we serve. Be part of an exciting and truly multi-disciplinary approach to patient care. Work alongside social workers, occupational therapist, psychiatrists, health care assistants and peer support workers all aiming to obtain the best recovery for our patients.

Main duties of the job

The CMHT is part of secondary care mental health services providing assessments and treatment for adult service users, with complex mental health needs.

As a CPN you will work collaboratively with the multi-disciplinary team (MDT) to achieve best mental health recovery for our patients.

You will be responsible for a caseload of patients of your own to support, guide and ultimately enable independence of the patients.

CPN's also conduct thorough assessments and complete clinical triages to support the team manage incoming referrals.

CPN's participate in the Duty Rota where you act as single point of contact for patient enquiries on the day.

Person Specification Experience Essential
  • Experience in mental health nursing.
  • Experience in the delivery of psychosocial/psychologically informed treatments and or CBT approaches.
  • Effective problem solving, decision making and demonstrates high standard of verbal and written communication skills.
  • Ability to effectively assess and manage risk.
  • Ability to work effectively as part of a team and autonomously.
  • Passionate about helping patients to realise their recovery goals.
Qualifications Essential
  • Relevant profession qualification i.e. RMN, or equivalent.
  • Evidence of post basic training and development.
