An opportunity has arisen for a 2 Band 6 Registered Mental Health Nurses, Social Workers or Occupational Therapists to join us in supporting individuals in acute mental health crisis at home, as well as supporting their loved ones during this time. We would also consider supporting preceptees at a Band 5 level to meet a number of competencies and achieve a substansive Band 6 role over a 12 month period.

Staff wellbeing and development is paramount - full supernumery induction is provided for the first 4 weeks in post. You would be supported with your professional development as part of your appraisal , and regular 1:1 supervision is offered as standard.

Main duties of the job

- To provide comprehensive assessment of a service users mental state and social needs which would aid the completion of a collaborative care plan for community management.

- To provide comprehensive risk assessment, formulation and management as part of the service user's journey.

- To act as keyworker for a small caseload of service users, with support from the multi-disciplinary team.

- Cover the duty / shift coordination role on a regular basis.

- Liaison with other agencies involved in a service users care.

- Lead on the discharge or transfer of care for our service users to appropriate agencies.

- To support and supervise junior members of staff.

- The team operates a 24/7 service, therefore there will be a requirement to cover shift work - flexible working can and will be supported however possible.

Person Specification Qualifications Essential
  • Post-graduate degree in Mental Health Nursing, Social Work or Occupational Therapy
Desirable
  • Additional qualification or training relevant to the role
Experience Essential
  • Acute care experience on placement and/or post qualifying
Desirable
  • Post qualifiying experience in Mental Health
Skills Essential
  • Experience of risk assessment and care planning
Desirable
  • Supervision of junior staff and / or students
Knowledge Essential
  • Understanding of the functions of a Home Based Treatment Team
