An exciting opportunity to join with the Crisis Resolution and home treatment teams as they further develop the service.

We are looking for three deputy team leads, one per locality. The deputy leads will have responsibility for the coordination of home treatment for service users in their locality, as they progress through crisis services and inpatient settings.

They will ensure that they maintain knowledge of allocated service users, provide a link between Crisis services, CMHT's and the inpatient ward.

The role will encompass supporting the service user to plan their care pathway, including facilitating early discharge from wards, and making that safety/follow up plans are in place before discharge from service, reviewing documentation (particularly care plans/risk assessments) and keeping things up to date.

It will be important to nurture locality relationships, to explore and make use of relevant 3rd sector organisations , collate feedback from Service users and carers on discharge, using the date to improve services and outcomes.

Main duties of the job

The post holder will ensure that the team provides therapeutic care within a holistic philosophy for those experiencing mental health problems and who are in acute crisis.

They will hold responsibility for a designated Locality providing; coordination of home treatment, holding knowledge of this Localities service users, a point of contact to CMHT/inpatient ward for this Locality, planning service user's pathway including facilitating early discharge from wards, ensuring that safety/follow up plans are in place before discharge from service. Nurturing locality relationships will be key in this post, including greater utilization of 3rd sector to aid longer term recovery.

The post holder is responsible for care delivery and maintaining standards of care and clinical practice in liaison with the wider multi-disciplinary team. This will include reviewing documentation (particularly care plans/risk assessments) and collating feedback from service users and carers on discharge, sharing learning from this feedback and implementing improvements based on it.

The ability to speak Welsh is desirable for this role; Welsh and/or English speakers are equally welcome to apply.
