This is a full-time (1.0 WTE) Consultant Psychiatrist post providing clinical and medical leadership to Ceddesfeld Ward. Ceddesfeld Ward is a 15-bedded assessment and treatment unit for male patients experiencing organic brain disease (the vast majority of which will be dementia-related) and the associated complications of these. The ward is co-located with Hamsterley Ward (a similar 15-bedded ward for female patients) on the Auckland Park Hospital site which has separate consultant psychiatrist input from Dr Sarah Pearce. Together, the two wards work closely together to provide the hub of organic inpatient care to patients in the Durham & Darlington locality and have a strong reputation regionally for high quality patient care. This post has arisen following the retirement of the previous longstanding post-holder.

Main duties of the job

The post-holder will be based at Auckland Park Hospital where Ceddesfeld Ward is located. Both Ceddesfeld and Hamsterley Wards are adjoining wards on the ground floor of the hospital and provide inpatient services to older patients with organic mental illnesses (most commonly due to dementia). Though the wards have separate nursing teams, there is a strong tradition of collaborative working and many members of the multidisciplinary team (MDT) work across both wards. The post-holder will be the consultant psychiatrist for Ceddesfeld ward and will provide the clinical and medical leadership to the multidisciplinary team and will be the consultant psychiatrist for the 15 beds on the ward. Admissions to the ward come via the community mental health teams, liaison teams or crisis teams and the vast majority of patients will be over the age of 65 years old. We are, however, a needs-led service, and sometimes younger patients may be admitted to the ward if tit best suits their needs (for example, patients with young-onset dementias). Ceddesfeld and Hamsterley Wards are assessment and treatment wards and so the vast majority of patients are discharged into the community (either to their own homes or into social care settings). T

Job description Job responsibilities

The Clinical PAs are 7.5 PA across the week and will be based on Ceddesfeld Ward. The consultant psychiatrist will provide clinical and medical leadership to the MDT and will lead the assessment and treatment of those patients on the ward. The post holder will be expected to work with a well-established, cohesive and enthusiastic MDT to maintain a high standard of clinical care for our patients. The ward operates a report out every weekday morning, which is effectively a board round of all patients on the ward. This daily meeting helps the MDT review the progress and risks of every patient over the preceding 24 hours, provides an opportunity for the MDT to discuss treatment plans and allocate any tasks. The consultant psychiatrist is expected to attend this meeting wherever possible. They are required to review any new admissions to the ward to determine the holistic treatment strategy and then continue to review these patients regularly to monitor their progress. They are also expected to attend any relevant patient meetings (e.g. Admission/Discharge Planning Meetings, CPAs, etc.) and maintain good relationships with patients families and carers in line with the principles of the Triangle of Care. The consultant psychiatrist will also assess and treat their patients in line with evidence based clinical; practice and in keeping with the Trust clinical pathways (with specific reference to the Dementia and Behaviours that Challenge pathways). They will model good prescribing practices for this patient group in line with national guidance. The consultant psychiatrist will be the named clinical supervisor for a Foundation Year 2 Doctor and will be additionally supported by a Physical Healthcare Practitioner. The Clinical Psychologist in the service is in the process of completing their non-medical Approved Clinician Training, at which time there is potentially scope to consider how this can be integrated on the ward so as to further support the inpatient role.

The post holder will be expected to have a familiarity with important relevant legislation affecting the care of older people with mental illness. The post-holder is expected to be the Responsible Clinician for any patients on the ward and will undertake any such duties that are specified by the Mental Health Act 1983 and in the accompanying Code of Practice (including, but not limited to working with the SOAD service, and preparing for and attending Mental Health Tribunals). The post-holder is expected to prioritise Mental Health Act Assessments for their own patients during working hours and be a qualified DOLS Mental Health Assessor for any patients on their personal caseload. Occasionally, there may be ad hoc Section 49 Mental Capacity Act reports to prepare, and these are shared evenly amongst the consultant body.

The Trust is eager to support the development of a special interest and this is negotiable either at the time of employment or at the Annual Job Plan Review meeting which takes place with the Clinical Director (or Associate Clinical Director) and Head of the services (or Locality Manager).

The appointee is encouraged to contribute to the education and training of medical students, doctors in training and other members of staff (including Physician Associates). There is weekly Locality teaching programme every Thursday afternoon held virtually over Microsoft Teams. Opportunities for formal involvement in the undergraduate and postgraduate teaching programmes are available through discussion with the Medical Development Department. There are a number of formal faculty roles which specialty doctors can apply for. In particular, doctors are encouraged to develop elective placements in psychiatry for 4th year medical students (known as student selected components or SSCs). Medical staff can maintain academic links with a range of Universities. Research and teaching links are particularly strong with Universities of Newcastle and Durham.

The organisation runs postgraduate academic programmes, in-house teaching, and specialty doctor development programmes, which the post-holder will be encouraged to participate in. They are also encouraged to engage with audit and service evaluation exercises in line with their PDPs and revalidation requirements. They will be a member of a peer group, which provides an additional source of support and supervision.

The appointee will be expected to undertake the ordinary administration duties of a Consultant post including participation in professional audit, business planning and related activities. A contribution to the leadership, management and strategic direction of the team is seen as essential. Consultants take a major role in the planning and management of services. There are opportunities for professional development in medical management.

All Consultants are automatically members of the Trust wide Senior Medical Staff Committee and each clinical speciality is represented on the Trusts Governance and Assurance Committee.

All consultants will

Maintain good clinical records using Trusts electronic record system

Undertake administrative duties associated with the care of patients

Record clinical activity accurately and comprehensively

Maintain a revalidation portfolio and participate in annual appraisal

Undertake a job plan review with the Clinical Director and Head of Service annually

Maintain professional registration with the GMC and approval as an Approved Clinician and Section 12(2) approved doctor

Comply with Trusts agreed policies and procedures

It is expected that the post holder will undertake training in the TEWV Quality Improvement System and will utilise the knowledge and skills in their own work and promote them with other members of staff in the team and the acute Trust.

It is also expected that the post holder will provide leadership to the multi-disciplinary team alongside the modern matron, ward manager and other senior clinical staff, and contribute to service development and clinical governance and responsibility for setting and maintaining standards with a commitment to Quality Improvement. The post holder will also work alongside other team leaders in promoting high standards of clinical governance within the team, including learning from mistakes, complaints and serious incidents. In the rare and unfortunate event that the post holder themselves is affected by a complaint or serious incident, then additional support can be provided by a named colleague arranged by the medical directorate as well as by additional support through internal and external partners.

The Trust actively supports the involvement of the consultant body in regional and national groups subject to discussion and approval with the Clinical Director, and as necessary, the Medical Director and Chief Executive

From time to time it may be necessary for the post holder to carry out other such duties as may be assigned, with agreement, by the Trust. It is expected that the post holder will not unreasonably withhold agreement to any reasonable proposed changes that the Trust might make.
