Exciting opportunity to join a great team within the Older Persons Mental Health Services on Onnen Ward Cefn Coed Hospital.

The ward is a dementia assessment ward with organic and functional mental health illnesses.

The team are a great team and are very welcoming, caring and work within the Health Board Values of Caring for each other, Always Improving and Working Together

Main duties of the job

The post holder will be required to function as a member of the multidisciplinary team, assess individual patients' care needs and subsequently develop, implement and evaluate appropriate programmes of care, acting as Care Co-ordinator for specified patients. They will allocate duties to nursing staff during their span of duty in charge of the ward. The role will include the supervision and instruction of unqualified and junior members of nursing staff.
