Are you a passionate Registered Mental Health Nurse, Occupational Therapist or Social Worker who is keen to make a difference and looking for a new challenge?
This is a great opportunity for you to work in a small multidisciplinary team with a shared caseload as a Band 6 Community Mental Health Professional within Taunton Assertive Outreach Team.
Taunton Assertive Outreach Team is part of the Assertive Outreach Service in Somerset and we are welcoming applications from enthusiastic and creative professionals to join our supportive team.
Main duties of the job
The team provides intensive mental health support to individuals with complex, severe and enduring mental health difficulties in the Taunton area, working in a creative and dynamic way to deliver community care to patients with complex needs. The team focuses on supporting individuals who historically have struggled to engage with standard community mental health support and are at high risk of relapsing or admission into inpatient services.
You will be working as a senior clinical member and delivering care for individuals within a shared caseload in the team. You will have a key role in supporting other members of the team in the development and delivery of mental health care in the community.
You will be committed to a recovery approach and have the ability to be flexible and work as part of a team.

To provide mental health assessment and planned care to service users with serious mental health difficulties. This will involve managing a caseload, multi-disciplinary working and arranging regular reviews of care. This will include, where appropriate, sharing information with carers/relatives and care coordinators and working with them in the process of assessment and the development and implementation of the clients care plan.

- Relevant professional qualification within Mental Health
- Evidence of recent relevant training
- Post qualification experience in mental health
- Working with clients in their own homes.
- Ability to work independently and as part of a team
- Knowledge of current developments in community mental health and assertive outreach
- Psychosocial intervention skills
- Speaks to distressed patients, and their relatives, by telephone phone and in person.
