  • To support the Chief Pharmacist in the provision of a specialist, safe, efficient and professional pharmacy service to community mental health teams involved in the provision of mental health services to adults within Barnet, Enfield and Haringey Mental Health Trust.
  • To help facilitate the management and treatment of patients living in North Central London experiencing a mental health illness.
  • To support the community mental health teams with Medicines Optimisation to prevent hospital admission and wherever possible facilitate transition safely into primary care.
  • To provide expert pharmacy and medicines management advice as well as advising on appropriate management strategies.
  • To promote patient safety, identify and mitigate medication-related risks in the Community.
  • To promote the development of services in line with the Enablement Clinical strategy and Medicines Optimisation Strategy.
  • To demonstrate specialist subject matter expertise in mental health related pharmacotherapeutics and to put this to use as a prescriber or in support of prescribing


  • Main duties of the job

    • To be an integral member of the community mental health pharmacy team and provide medicines management services.
    • To work with members of the community mental health teams to provide safe, timely, appropriate and high-quality pharmaceutical services for community based patients.
    • To work with the multidisciplinary team, third sector organisations, patients and carers to improve service delivery, experience and outcomes for patients who take medications for a mental health condition.
    • To ensure safe transition of care between interfaces ensuring that medication changes are communicated to all teams and services involved in the patient's care.
    • Ensuring that patient's physical health is considered as part of a holistic package of care ensuring that the impact of interactions between physical health and mental health conditions and treatments is reviewed and that appropriate monitoring is undertaken.
    • Working with clinics & PCN staff to develop & deliver initiatives to improve the uptake and utility of physical health checks for patients managed in the community.
    • Provide highly specialist medicines management support and training for nurses and other members of the multi-disciplinary team across the interface where appropriate.
    • Support the collection of audit/QI data associated with monitoring outcomes of the service provided.

    Person Specification EDUCATION AND QUALIFICATIONS Essential
    • Professional registration with the General Pharmaceutical Council
    • Master's degree in pharmacy or equivalent
    • Completion of pre-registration experience
    Desirable
    • Certificate or Diploma in psychiatric Pharmacy practice
    • Credentialed Member of College of Mental Health Pharmacy or in the process of applying for Royal Pharmaceutical Society Faculty Membership
    EXPERIENCE AND KNOWLEDGE Essential
    • Experience of dealing with patients where language/understanding or learning difficulties may be a barrier to effective communication
    • Knowledge of medicines used in mental health
    • A minimum of 2 years post-qualification experience as a pharmacist.
    Desirable
    • Experience of training or supervising staff
    • Experience of management or a leadership role
    • Experience or involvement in Clinical Trials
    SKILLS AND ABILITIES Essential
    • Effective written and verbal communication skills and good interpersonal skills - with pharmacy team members, public and other health professionals.
    • Can demonstrate an understanding of the key stakeholders the post is likely to be supporting and how they are expected to relate to them.
    • Ability to demonstrate attention to detail, accuracy, numeracy/calculation skills.
    Desirable
    • Experience in using ePMA
