The role involves care co-ordination of clients within the CMHT/OP service to provide high quality care.
Following a redesign of services, we are in an exciting position to be able to recruit two Senior Community Mental Health Nurses for our Older Adult Services in the Southend & Castlepoint locality - primarily your caseload would be working either within Southend or Castlepoint Locality. This is a supportive, well-resourced multi-disciplinary team with Support Workers, Physical Health Practitioners, Qualified Nursing and Medical input. The team has a friendly atmosphere and strong work ethic.
We are seeking two RMNs who are looking to be part of future healthcare and integrated working for the older adult population and supporting them to live independently.
Main duties of the job
As a Senior Community Mental Health Nurse within our Older Adult Service and key member of the team, you will be responsible for managing a defined caseload providing assessment and coordination of care needs, planning, implementation and evaluation of evidence-based nursing care for service users and their carers
As a Senior member of the team, you will need to attend the MDT meetings and to provide professional advice, including health/mental health promotion and support to members of the service as well as other agencies.
You will be required to assist the Clinical lead in their absence as appropriate and as a role model and resource for less experienced members of the team, including providing supervision.
You will also need to have the ability to work under pressure and although a key part of the team, on your own initiative.

- act as an independent practitioner within a defined community setting, while liaising with other professionals as necessary
- have a working knowledge of local, national policies and legislation, which govern current service provision and audit
- ensure effective communication pathways exist
- be responsible for ensuring the promotion of equality diversity and rights in accordance with good practice and legislation
- be responsible for identifying and contributing to the training needs necessary in providing quality evidence-based care as appropriate.
