An exciting new opportunity has arisen for a Band 7 Non Medical Prescriber with the North East Derbyshire PCN. As part of the NHS Long Term Plan and working alongside DHCFT Living Well Service we have an innovative and unique role integrated within Primary Care Networks.

You will be able to help educate and support teams within primary care.

Main duties of the job

The North East Derbyshire PCN services are creating a new mental health hub team this is being developed to improve capacity within the PCN Network and to provide expert intervention to better manage patients presenting with mental health problems within primary care.

The service provides appointments for North East Derbyshire GP registered patients. The role will provide opportunity to work within the Primary Care Setting and alongside DHCFT Mental Health and the Living well project.

You will have opportunity to help shape the services functionality, outcomes and the overall outlook of the new integration. This support will build the confidence of staff in managing mental health, will build the resilience of individual members of staff and improve quality.

Your role will involve supporting the other Mental Health Health Practitioners; offering supervision , ensuring suitable training opportunities . You will also be able to support the service delivery and support the measurement of the effectiveness of the service . Your role may also be required to support with triage and support in management of complex cases.

You will work both autonomously and as part of a wider team.

Job description Job responsibilities

You will consult with patients within the NED Practices; this will be a combination of remote consultations and face-to-face depending on need.

Your role will be a combination of supporting with triage and managing your own intervention consultations as well as reviewing medications effectiveness and side effects , physical effects (and tasking GP as necessary) and prescription of suitable medication.

You will consult, taking a brief, focused history and conducting a risk assessment, formulation of need and onward plan.

You will work both autonomously and as part of a wider team.

You will also be required to offer supervision to the DHCFT PCN B6 Mental Health Practitioners

You will also have opportunity to support general practice staff within the NED primary care networks. This support will build the confidence of staff in managing mental health, will build the resilience of individual members of staff and improve quality of the service.

Person Specification Attainment/ Qualifications Essential
  • Registered Nurse ( Mental Health Nurse )
  • ENB 997/8, NVQ Assessor, CG 730 or equivalent
  • NMP Qualification
Desirable
  • Evidence of post-graduate training in mental health
  • Leadership Qualification
Knowledge/ Experience/ Skills Essential
  • Demonstrable experience in working with Adults of working age with mental health difficulties ,(as a guide equivalent to 3 years full time) post registration clinical expertise that will includes core competencies around N.M.C. Nurse or other professional equivalents.
  • Demonstrate the ability to co-ordinate and lead multi-professional teams.
  • Demonstrate leadership skills such as motivation, facilitation, mentorship.
  • Demonstrate up to date evidence-based knowledge for Professional Practice generally and the chosen specialty.
  • Demonstrate the ability to lead the clinical team in a service user / career focused way.
  • Demonstrate exposure to and a comprehensive knowledge of Clinical Governance in action (audit, research clinical effectiveness etc).
  • Demonstrate the ability to deliver Clinical and Managerial Supervision within an effective model.
