We have a brand-new opportunity for a Mental Health Nurse, Social Worker or Occupational Therapist with mental health experience to join the Crisis Resolution & Home Treatment Team (CRHT) at Langley Green Hospital, Crawley.

The CRHT team is a part of acute care and works closely with the acute hospital and community teams to support clients in a mental health crisis. It offers clients care and support in their own homes together with a day service group programme based at Langley Green Hospital as an alternative to hospital admission.

CRHT are able to offer all the benefits of a hospital admission while remaining at home including home visits from our experienced team, with support from doctors and psychology. We also facilitate supportive discharge from hospital, managing the transition back to community-based service.

Main duties of the job

As a Community Mental Health Practitioner in our team you will need to actively work together with service users, carers and a wider multi-disciplinary team to help formulate care plans and utilise appropriate assessments.

You will be working in a variety of community settings to provide and improve person-centred care of the highest quality. You will be directly involved in developing discharge plans in collaboration with service users and their families, as well as occasionally representing the clinical team at multi-disciplinary meeting and clinical reviews of patient care.

You will also be responsible for monitoring and developing treatment plans in order to make progress with service users and ensure effectiveness of interventions.

REQUIREMENTS

In order to be a successful Community Mental Health Practitioner in our team you will have:
  • Professional Qualification
  • Experience of working within an acute setting
  • Experience of working with people with complex needs
  • Sound evidence-based knowledge of treatment pathways

It would also be advantageous if you have:
  • Experience of working within a Community setting


The service operates 7 days a week and there will be an expectation to work weekends and shifts, which would involve early and late shifts.

Please note you will need to have a current driver's license and access to a car as the role requires regular home visits to service users and occasional visits to other trust sites.
