An exciting opportunity has arisen to join our innovative Advance Care Planning Team, supporting patients on the District Nursing caseload to receive person centred care in line with their wishes and preferences as they near the end of life.

The post holder will be working to support patients with advanced dementia and those who are important to them, to take part in discussions around their future care. They will be working closely with a range of other services to provide personalised and well co-ordinated care for patients as they near the end of life.

Main duties of the job

They will also have opportunities to work with patients who have a learning disability or learning difficulty, supporting them to express their preferences in relation to their future care, and ensuring that reasonable adjustments are in place to enable equitable access to care.

This is an opportunity to join a team who are passionate about supporting equitable access to end of life care and who were shortlisted last year for a HSJ award.

Person Specification Education/ Qualification/ Training Essential
  • oRegistered Mental Health or Learning Disability Nurse with evidence of current NMC registration
  • oRelevant post-graduate short specialist courses at MSc level or working towards this
  • oEvidence of continuing professional development
  • oPalliative or end of life care course
  • oLeadership training
  • oSupporting learning and assessment in practice course or equivalent
Experience Essential
  • oRelevant experience of working with patients with advanced dementia and / or learning disabilities who may be nearing the end of life.
  • oExperience of integrated care and of liaising effectively with other organisations
  • oExperience of talking about advance care planning including death & dying
  • oRelevant leadership experience
  • oExperience of working in community settings.
  • oExperience of working in a multicultural environment
Knowledge and Skills Essential
  • oKnowledge and understanding of the principles of advance care planning.
  • oExcellent interpersonal and communication skills
  • oAbility to oversee and deliver a high standard of evidence based nursing care
  • oAble to work autonomously
  • oAbility to develop effective relationships with partners from specialist palliative care, primary care and social care
  • oEffective teaching and coaching skills.
  • oComputer literate
  • oExperience of quality improvement
Other Essential
  • oHigh level of self motivation
  • oAble to professionally and personally develop oneself and other team members
  • oAble to work shifts covering 8am-8pm rota
  • oCar Driver
