Surrey and Borders Partnership NHS Foundation Trust is working with our Integrated Care Systems to undertake a once in a generation reworking and shift in community mental health service provision for the people of Surrey and North East Hampshire. Working across health, social care and Voluntary, Community and Social Enterprise (VCSE) sector partners as well as our diverse community and service users, their carers and their families to co-produce and implement an innovative new model of personalised care set out in the NHS Community Mental Health Framework for Adults and Older Adults.

Every community mental health service in England is changing in line with this new vision set out in the NHS Long Term Plan – and we’re in the vanguard of change as an ‘early implementer’ site.

This role involves both variety and complexity of working in a rapidly evolving programme of whole system change. We are not looking for more of the same but to continue our shift in approach to transformation and this presents a fantastic opportunity to be part of our radical journey as we look to appoint an experienced Senior Community Mental Health Practitioner.

Main duties of the job

This is a new, exciting and innovative position that will provide expert support to our Community Mental Health Recovery Services (CMHRS) as they prepare and mobilise towards our ‘One Team’ approach for fully integrating with new primary care based mental health teams, formed around local Primary Care Networks (PCNs). You will be responsible for providing short term specialist mental health advice and education to primary and secondary care colleagues, as well as providing clinical leadership, in conjunction with the Service Manager, to deliver a thorough caseload review. You’ll also use your expertise to implement strategies in both primary care and mental health that promotes an environment of user involvement and recovery.

We are looking for autonomous, driven, solution-focused and confident practitioner(s). Job description Job responsibilities

Job Overview

The post holder will work as part of a roving team, to prepare and mobilise Community Mental Health Recovery Services to integrate into one team with primary care mental health services in line with the Community Mental Health Transformation Framework.

The post holder will provide short term specialist mental health advice and education to primary and secondary care colleagues. The post holder will provide clinical leadership, in conjunction with the Service Manager, to deliver a thorough caseload review.

Once reviewed, the post holder will provide leadership and holistic, autonomous nursing care to action the results of the caseload review.

In so doing, the post holder will maintain communication with primary care professionals and other professionals and agencies through regular informal and formal meetings.

The post holder will facilitate clinical sessions and assess / triage clients/patients according to agreed assessment tools, offering short-term interventions based on psychological therapies and offer professional advice to client / patients and their carers, other professional, statutory and voluntary agencies.

The post holder will facilitate the transfer of patients between primary and secondary care and between the different specialist teams within the Trust and to care coordinate clients on both standard and enhanced CPA.

The post holder will be responsible for their own nursing practice in accordance with Nursing and Midwifery Council Guidelines and will take every reasonable opportunity to sustain their professional competence.

The post holder will provide clinical leadership for the nurses in their specific area of practice, providing advice on all nursing issues and to act as a resource person for other professionals within that service. The post holder will identify and facilitate nurses updating their professional knowledge and skills.

In the absence of the Service Manager the post holder will be responsible for delegating duties to other members of the team and taking a lead on the day to day running of the team, undertaking management responsibilities as requested.

Principal Duties:

The post holder will:

1. In conjunction with the Service Manager undertake a review of current caseload, segmenting case load according to pre-defined needs and pathways.

2. In conjunction with the Service Manager, coordinate actions, interventions and care arising from the caseload review.

3. Implement strategies in both primary care and mental health that promotes an environment of user involvement and recovery.

4. Act as a role model for colleagues and others in relation to professional and personal conduct and practice.

5. Ensure implementation of agreed standards of nursing care.

6. Ensure each client / patient has a defined individual care programme to meet their needs.

7. Ensure all policies and procedures relevant to the role are implemented and complied with.

8. Ensure that regard is given to dignity, choice, self esteem and well-being of service users and carers.

9. Use nursing interventions in a variety of setting to engage with service users in group work and individual programmes, using community resources where possible.

10. Ensure that agenda for social inclusion is reflected in interventions offered and implemented.

11. Discharge professional responsibilities under Surrey Multi-agency Child Protection and Safeguarding Adults Procedures, the Mental Health Act 1983, the Mental Capacity Act 2005 and other relevant legislation.

12. Participate in, and lead on specific area’s of Health and Social governance relevant to the post.

13. Use the Trust’s IT system as and where appropriate.

14. Collate evidence of own continuous updated clinical knowledge and skills and ensure junior staff do likewise through the supervision process.

15. Carry out regular clinical auditing of nurse documentation and evaluation of clinical practice, auditing of Trust Services as expected and required.

16. Continue to comply with the Nursing and Midwifery Council and ensure the Guidelines on accountability, confidentiality and administration of medication and other Guidelines that may apply from time to time are adhered to.

Key Clinical Duties and Responsibilities

1. Responsible for review of the respective Community Mental Health Recovery Service’s caseload.

2. Responsible for a defined caseload within the team arising from the caseload review. Using specialist Mental Health knowledge and skills, carry out individual nursing assessment, plan and formulate a range of interventions and treatment options within care plans, implement and evaluate packages of care.

3. Undertake the task of care coordinator, organise CPA review meetings, discharge planning meetings involving service users, their carers, other professionals and agencies, attend inpatient reviews and act as client advocate if necessary.

4. Facilitate the development and use of care pathways by the multi-disciplinary team.

5. Treat clients with short term interventions where assessment has indicated this. Assess and treat clients with complex, severe and enduring mental health problems who may be on a community section of the mental health act.

6. Facilitate the transfer of client from Primary Care to Mental Health Services as appropriate and discharge clients from case load to appropriate agencies when nursing interventions are completed.

7. Act as a support and specialist advisor to GP’s and primary care colleagues as appropriate.

8. To facilitate and develop with Primary Care Staff, joint education and training programmes of mental health issues in primary care.

9. To be part of the Senior Staff Group and attend relevant regular meetings as scheduled.

10. Ensure that effective communication systems exist between the team and Primary Care.

11. Ensure information is collected in such a way that it conforms to the Trust’s and Social Care quality data required.

12. Carry out agreed nursing procedures and clinical treatments in accordance with agreed policies.

13. To attend Tribunals and other Multi-Disciplinary Meetings and prepare reports.

14. To supervise and participate in the safe carriage, storage and delivery of medication to clients both in the clinical area and domestic environment, according to Trust and National Guidelines and Policies, ensuring that ordering and delivery of clinical equipment meets policy requirements.

15. To act as mentor or delegate appropriate mentor for nurses from other clinical areas wishing to gain experience in Team.

16. To carry out Carer’s Assessments in accordance with the Carer’s Act.

Education / Research / Audit

The post holder will:

1. Act as a health educator for patients, relatives, carers and staff. Advise on issues of physical and mental health so as to engender a positive approach to health and strengthen the patient’s capacity to manage her/his illness, symptoms and crises.

2. Implement and participate in orientation and induction programmes for all new staff.

3. Facilitate and participate in continuing education programmes to enhance the services offered to clients and staff.

4. Be responsible for identifying her/his own training and developmental needs in consultation with their Professional Supervisor and line manager.

5. Be responsible for developing, implementing and monitoring standards of nursing care within the service and ensuring that they comply with the Nursing and Midwifery Council’s Code of Professional Conduct, the curriculum of Universities whose students undertake placements with the service.
